摘要
目的:探讨胫骨横向骨搬移治疗老年患者足部慢性溃疡的临床疗效。方法:选取慢性足部溃疡的老年患者12例为研究对象,均予胫骨横向搬移治疗,术后随访患者直至溃疡创面愈合,统计患者足部溃疡愈合率,比较术前1 d与术后30 d的足背部皮温、踝肱指数、血清炎性反应相关指标[C-反应蛋白(CRP)、红细胞沉降率(ESR)、白细胞计数(WBC)]。结果:12例患者的慢性溃疡愈合11例,愈合率为91.7%,愈合时间为12~18周,平均愈合时间为(13.2±1.3)周。术前1天与术后第30天的足背部皮温、CRP、ESR、WBC、踝肱指数均明显改善,其中CRP、ESR、WBC下降尤为显著,差异有统计学意义(P<0.05)。结论:胫骨横向搬移术后,患者的局部慢性炎性反应得以控制,局部血管、神经以及组织均得到有效改善,胫骨横向搬移术治疗老年患者足部慢性溃疡疗效显著,值得临床推广。
Objective To investigate the clinical effect of lateral bone transport of tibia in the treatment of chronic ulcers in elderly patients.Method Twelve elderly patients with chronic foot ulcers were enrolled in the study.All patients were treated with transverse movement of the tibia.The patients were followed up until the ulcer wound healed.The rate of foot ulcer healing was counted.The skin temperature of the back of the foot and the index of sputum,C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),and white blood cell count(WBC)were compared between 1 day before surgery and 30 days after surgery.Results 12 patients had chronic ulcer healing in 11 cases,and the healing rate was 91.7%,the healing time was 12-18 weeks,and the average healing time was(13.2±1.3)weeks.The skin temperature,CRP,ESR,WBC,and sputum index of the foot and back were significantly improved 1 day before and 30 days after surgery,and the CRP,ESR,and WBC decreased significantly,the difference was statistically significant(P<0.05).Conclusion After the lateral movement of the tibia,the local chronic inflammation of the patient was controlled,and the local blood vessels,nerves and tissues were effectively improved.The transverse movement of the tibia was effective in treating chronic ulcers in the elderly patients,which is worthy of clinical promotion.
