摘要
目的:探讨基于加速康复理念的全程个案管理模式在乳腺癌手术患者中应用的效果。方法:选取2017年7月1日~8月31日收治的乳腺癌改良根治术患者60例纳入对照组,采用乳腺癌常规护理;选取2018年2月1日~3月31日收治的乳腺癌改良根治术患者60例纳入观察组,采用基于加速康复理念的全程个案管理模式。在入院时、出院时、术后第3个月及第6个月4个时间点对两组患者的住院时间、引流管留置时间、并发症发生率、患肢功能恢复效果及患者生存质量进行评估。结果:观察组引流管留置时间、住院时间均短于对照组(P<0.01),术后并发症发生率低于对照组(P<0.05),患肢功能恢复效果优于对照组(P<0.05,P<0.01);观察组在出院时、术后第3个月及6个月时生存质量评分均优于对照组(P<0.01),两组患者生存质量在时间和干预方式交互作用有统计学意义(P<0.01)。结论:基于加速康复理念的全程个案管理模式可促进乳腺癌患者术后康复,缩短患者引流管留置及住院时长,降低术后并发症的发生率,促进患肢肩关节功能恢复,改善患者生存质量。
Objective:To investigate the effect of the whole-course case management model based on the concept of enhanced recovery after surgery in the breast cancer patients with surgery.Methods:A total of 60 patients with modified radical mastectomy from July 1 to August 31,2017 were included in the control group and routine care for breast cancer was used.A total of 60 patients with modified radical mastectomy from February 1 to March 31,2018 were selected in the observation group,and the whole-course case management model based on the concept of enhanced recovery after surgery was adopted.The length of hospitalization,the length of drainage tube indwelling,the incidence of complications,the function recovery effect of the affected limb and the quality of life of the patients at the time of admission,at the time of discharge,3 and 6 months after the operation in the two groups were evaluated.Results:The duration of drainage tube indwelling and hospitalization in the observation group was shorter than that of the control group(P<0.05),the incidence of postoperative complications was lower than that of the control group(P<0.05),and the recovery of limb function was better than that of the control group(P<0.05).The observation group was superior to the control group in terms of quality of life scores at discharge,3 months and 6 months after surgery(P<0.05),and the interaction between time and intervention mode of the two groups was statistically significant(P<0.05).Conclusion:The whole-course case management model based on the concept of enhanced recovery after surgery can promote the postoperative recovery of breast cancer patients,shorten the duration of drainage tube indwelling and hospitalization,reduce the incidence of postoperative complications,promote the recovery of shoulder joint function,and improve the quality of life of patients.
