摘要
目的探讨对胫骨平台后外侧塌陷骨折患者应用改良扩大外侧切口入路治疗后的效果观察。方法将2018年4月~2019年4月我院收治的44例胫骨平台后外侧塌陷骨折患者作为研究对象,采用随机数字表法分为对照组(传统前外侧入路治疗)和观察组(改良扩大外侧切口入路锁定钢板固定治疗),每组22例。对比治疗后患者的手术时间、术中出血量、骨折愈合时间及术后患者患侧和健侧膝关节后倾角及内翻角,对比术后HSS膝关节评分情况。结果治疗后,观察组患者的手术时间、术中出血量、骨折愈合时间、感染情况均优于对照组患者的相关指标;观察组患者的膝关节HSS评分总得分显著高于对照组患者的膝关节HSS评分总得分,差异有统计学意义(P<0.05)。治疗后,观察组患者与对照组患者患侧及健侧膝内翻角及后倾角数值比较,差异无统计学意义(P>0.05)。结论对于胫骨平台后外侧塌陷骨折患者,采取改良扩大外侧切口入路治疗,可有效改善临床相关治疗指标,提高膝关节活动能力,有较高临床应用价值。
Objective To explore curative effects of modified enlarged lateral incision approach in treatment of patients with posterior lateral collapse fracture of tibial plateau.Methods 44 patients with posterior lateral collapse fracture of tibial plateau who were admitted and treated in our hospital from April 2018 to April 2019 were selected as research objects.According to the random number table method,they were divided into the control group(traditional anterior lateral approach)and the observation group(modified enlarged lateral incision approach with locking plate fixation),with 22 cases in each group.The operative time,intraoperative blood loss,fracture healing time,postoperative posterior slope and varus angle of knee joint of the affected side and healthy side and HSS knee joint score of one year after surgery were compared between two groups.Results After treatment,the operative time,intraoperative blood loss,fracture healing time and infection in the observation group were all better than those in the control group.The total HSS knee joint score in observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,there was no statistically significant difference between the observation group and the control group in varus and posterior slope of the affected side and the healthy side(P>0.05).Conclusion For patients with posterior lateral collapse fracture of tibial plateau,modified enlarged lateral incision approach can effectively improve clinically relevant treatment indexes and improve knee joint activity,which has high clinical application values.
