摘要
目的:比较Mis-TLIF经皮空心椎弓根螺钉与常规椎弓根螺钉用于单节段退行性腰椎滑脱的疗效。方法:选取98例退行性腰椎滑脱患者,随机分为研究组(n=51)和对照组(n=47)。研究组使用Mis-TLIF经皮空心椎弓根螺钉治疗,对照组使用常规椎弓根螺钉治疗。比较两组患者手术时间、术中出血量、住院时间、椎体高度、椎间隙高度、Oswestry评分、生活质量评分、并发症情况及再手术情况的差异。结果:与对照组比较,研究组手术时间更短、术中出血量更少、术后住院时间更短,差异有统计学意义(P<0.05)。两组手术前后椎体高度、椎间隙高度、Oswestry评分、生活质量SF-36评分比较,差异无统计学意义(P>0.05)。术后,两组椎体高度、椎间隙高度比较,差异无统计学意义(P>0.05);研究组Oswestry评分低于对照组(P<0.05),SF-36评分高于对照组(P<0.05)。研究组螺钉松动发生率和再次手术率均低于对照组(P<0.05)。结论:Mis-TLIF经皮空心椎弓根螺钉治疗单节段退行性腰椎滑脱在诸多方面优于常规椎弓根螺钉,两种方法均对脊柱生物学性能影响小。
Objective:To compare the effect between Mis-TLIF percutaneous hollow pedicle screws and conventional pedicle screws for single-segment degenerative lumbar spondylolisthesis.Methods:98 cases of single-segment degenerative lumbar spondylolisthesis patients were divided into research group(51 cases,received Mis-TLIF percutaneous hollow pedicle screws treatment)and control group(47 cases,received pedicle screws treatment).The surgical time,intraoperative blood loss,hospital stay,vertebral height and intervertebral space,Oswestry score,SF-36 score,complications and reoperation of the two groups were compared.Results:Research group had shorter surgical time,less intraoperative blood loss,shorter hospital stay,compared with the control group,the difference was statistically significant(P<0.05).There was no significant difference in vertebral height,intervertebral space,Oswestry score,SF-36 score between two groups before and after surgery(P>0.05).There was no significant difference in vertebral height,intervertebral space after surgery(P>0.05).Research group had lower Oswestry score,higher SF-36 score after surgery(P<0.05).The complications rate and reoperation rate of research group were lower than these of control group(P<0.05).Conclusion:Mis-TLIF percutaneous hollow pedicle screws has better outcomes than pedicle screws for treating single-segment degenerative lumbar spondylolisthesis.Both the two methods have little effect on the biological performance of the spine.
