摘要
分析克兰费尔特综合征(KS)合并糖尿病患者的骨质疏松(OP)及骨折特点.患者男,53岁,因"骨折41年、口干3年"入院.患者分别于12岁、23岁、30岁及47岁时出现右小指、右胫骨上段及双侧踝部骨折,3年前因口干查FPG 10 mmol/L.入院查体消瘦体型,颜面无须,阴毛稀疏,睾丸3 ml.实验室检测血清睾酮2.24 nmol/L,促性腺激素高于正常值,HbA1c 15.3%,ICA、GADA、IAA阴性,腰椎总T值-4.4,染色体测定47,XXY.诊断为KS、特殊类型糖尿病、严重OP、陈旧骨折(小指、膝下、双踝).
To investigate the characteristics of osteoporosis(OP)and fracture in Klinefelter syndrome(KS)patients complicated with diabetes mellitus.A 53 years old male patient was admitted in hospital because of'Bone fracture for 41 years and thirsty for 3 years'.The patient had bone fracture in right fifth finger,right upper tibia and both an kies when he was 12,23,30 and 47 years old.Three years ago,because of thirsty,he tested fasting blood glucose,and the result was 10 mmol/L.Physical examination showed that he was thin,had no beard,sparse pubic hair and small testes.Lab tests showed a low testosterone level and high gonadotropic hormones levels.HbAiC was 15.3%.ICA,GADA and IAA were negative.Lumbar ver tebra T value was-4.4,and chromosome karyotype was 47,XXY.He was diagnosed as KS,diabetes mellitus,severe osteoporosis,and fractures(right fifth finger,right upper tibia and both ankles).
