摘要
目的:探讨斜外侧入路椎间融合术(OLIF)治疗腰椎退行性疾病的效果及手术护理配合。方法:选取2018年1月~2019年3月在我院接受OLIF治疗的58例腰椎退行性疾病患者为研究对象,比较OLIF手术患者术前和术后3天VAS评分、ODI指数、脊柱生活量表评分。结果:在VAS评分、ODI指数、脊柱生活量表评分方面,术前分别为(4.78±1.18)分、(30.81±8.40)分、(65.31±7.65)分,术后3天随访分别为(2.14±0.86)分、(20.88±7.27)分、(52.40±9.47)分,术后VAS评分、ODI、脊柱生活量表评分方面均低于术前,差异有统计学意义(P<0.05)。发生15例并发症(25.9%),其中6例为术中并发症,发生率为10.3%,腰椎节段动脉损伤2例(3.4%),处理后未发生大出血;终板损伤3例(5.2%),腹膜损伤1例(1.7%);9例为术中并发症(15.5%),左侧交感神经损伤3例(5.2%),左侧大腿前侧疼痛1例(1.7%),左侧股四头肌无力2例(3.4%),融合器下沉3例(5.2%)。结论:OLIF治疗腰椎退行性疾病可降低患者疼痛和功能障碍,提高患者的生活质量。
Objective:To investigate the clinical outcomes and roles of nurse cooperation in patients with lumbaRdegenerative disease treated by oblique lateral lumbaRinterbody fusion(LLLIF).Methods:Fifty-eight patients with lumbaRdegenerative disease undergoing OLLIF in ouRhospital were enrolled from January 20 to March 2019.The clinical data,including scoring by visual analogue scale(VAS),Oswestry Disability Index(ODI)and quality of life scale foRChinese patients with degenerative spine disease(QLS-DSD),were compared before and 3 days afteROLLIF.Results:Scoring by VAS,ODI and QLS-DSD was 4.78±1.18,30.81±8.40 and 65.31±7.65 before operation,and 2.14±0.86,20.88±7.27 and 52.40±9.47 by follow-three days following surgery,respectively,which was loweRafteRoperation than before operation(P<0.05).Complications occurred in 15 patients(25.9%),including intraoperative complication in 6(10.3%).lumbaRsegmental artery injury in 2(3.4%),endplate injury in 3(5.2%)and peritoneal injury in 1(1.7%).Postoperative complications were seen anotheR9 patients(15.5%),in whom 3(5.2%)were associated with left sympathetic nerve injury,1(1.7%)was with pain in left thigh,2(3.4%)were with transient left quadriceps and 3(5.2%)with fusion cage subsidence.Conclusion:OLLIF can reduce the pain and improve the dysfunction and life quality of patients with lumbaRdegenerative diseases.
