摘要
痛经主要责之于肝肾冲任,与肝关系最为密切;"瘀"是本病的关键病机,乃胞门经道弛张开缓失司,经水瘀滞下而不畅所致,气血阻滞为主,临床以寒凝、气滞血瘀较为常见。疼痛发作期,治疗宜"活、行、温、通"四法并用,兼顾标本虚实,辨证施治。根据女性特殊生理,采用分阶段论治原则,经前及经期以活血化瘀、行气通滞为主,经后以补肾疏肝,调理冲任为主,且遣方用药少用温燥之品,以防药物过于温燥而伤血。胡教授经验效方"经痛舒"方针对胞脉瘀滞病机,以活血行气,通经止痛为主,使寒温并用,寒邪得散,冲任瘀滞得除,再兼以经期红糖、黄酒为引的特殊服药方法,痛经自止。
Dysmenorrhea is mainly responsible for liver and kidney,which is most closely related to liver."Blood stasis"is the key pathogenesis of the disease.It is caused by dysfunction of relaxation and opening of the uterine ostium channel,and the stagnation of menstruation.It is mainly caused by the block of qi and blood.Clinically,cold coagulation,qi stagnation and blood stasis are common.In the stage of pain attack,the treatment should be combined with the four methods of"Huo,Xing,Wen and Tong",tak-ing into account the deficiency and excess of the manifestation and root cause,syndrome differentiation and treatment.According to the special physiology of women,the principle of treatment by stages was adopted.Before and after menstruation,it was mainly for promoting blood circulation and removing stasis,promoting qi and clearing stagnation.After menstruation,it was mainly for tonify-ing the kidney and liver,regulating the Chong and Ren,and the prescription was used less for warming and dryness,so as to pre-vent blood injury caused by excessive warming and dryness.According to the pathogenesis of stagnation of uterine Channels,Pro-fessor Hu's empirical prescription"Jingtongshu"is a special method of taking medicine,which mainly promotes blood circulation and qi circulation,promote menstruation and relieve pain,and uses cold and warm medicine together to disperse pathogenic cold,remove stasis of Chong Ren and Ren,and also uses brown sugar and yellow wine as medicine guide during menstruation.In this way,dysmenorrhea stops itself.
