多黏菌素耐药的重要肠杆菌科细菌耐药及表型分析

摘要 目的研究温州地区临床分离肠杆菌科细菌对多黏菌素耐药性,分析多黏菌素耐药肠杆菌科细菌耐药表型及流行特征,为当地临床的感染防控提供指导。方法采用肉汤稀释法测定菌株对多黏菌素耐药性,采用E-test法测定多黏菌素耐药株对其他抗生素的敏感性。结果在528株大肠埃希菌中,共检出多黏菌素耐药6株,阳性率为1.14%。420株肺炎克雷伯菌中,共检出多黏菌素耐药株3株,阳性率为0.71%。82株阴沟肠杆菌无多黏菌素耐药株。此9株多黏菌素耐药株对头孢菌素、氨曲南及喹诺酮类敏感率较低,对亚胺培南、美罗培南、替加环素敏感率高。4株多黏菌素耐药大肠埃希菌同时产ESBLs,产ESBLs同时耐碳青酶烯类多黏菌素耐药肺炎克雷伯菌1株。结论多黏菌素耐药株在温州地区未形成流行趋势,呈散发。但已经发现与产β-内酰胺酶和碳青酶烯耐药共存的现象,提示临床应加大多黏菌素耐药情况的监测力度。 Objective To study the resistance of bacteria to polymyxin in the clinical isolates of Enterobacteriaceae in Wenzhou,and to analyze the drug-resistant phenotype and epidemic characteristics of the polymyxin-resistant Enterobacteriaceae,so as to provide guidance for the prevention and control of the local clinical infection.Methods Broth dilution method was used to determine the resistance of strains to polymyxin,and E-test method was used to determine the sensitivity of polymyxin resistant strains to other antibiotics.Results Among 528 strains of Escherichia coli,6 strains of polymyxin resistance were detected,the positive rate was 1.14%.Among 420 strains of Klebsiella pneumoniae,3 strains of polymyxin resistant strains were detected,the positive rate was 0.71%.82 strains of Enterobacter cloacae were non-polymyxin resistant strains.The sensitivity rate of these 9 polymyxin resistant strains to cephalosporins,aztreonam and quinolones was low,and that to imipenem,meropenem and tegacycline was high.4 strains of polymyxin resistant Escherichia coli produced ESBLs and 1 strain resistant to carbapenem polymyxin and Klebsiella pneumoniae was observed.Conclusion Polymyxin resistant strains in Wenzhou area have not yet formed an epidemic trend,showing sporadic.However,the coexistence of β-lactamase and carbocylase resistance has been found,suggesting that the monitoring of polymyxin resistance should be increased in clinic.
