摘要
目的研究老年人乳腺癌病理特征及预后影响因素分析。方法收集2010年3月至2013年3月我院诊治的老年女性乳腺癌病人217例,根据年龄分为65~75岁组(n=145)和>75岁组(n=72),观察2组病人临床病理特征,并对5年总生存时间(OS)、无病生存时间(DFS)进行单因素和多因素分析。结果2组病人在合并症、肿瘤大小、淋巴结转移、病理类型、临床分期及人表皮生长因子受体-2(HER-2)表达方面差异有统计学意义(P<0.05)。多因素分析显示,影响老年乳腺癌病人5年OS的因素有:年龄、淋巴结转移和孕激素受体(PR)表达(P<0.05),影响老年乳腺癌病人5年DFS的因素有:年龄和淋巴结转移(P<0.05)。结论老年人不同年龄段乳腺癌临床病理特征存在差异,影响预后的因素主要为年龄、淋巴结转移、PR、组织学特点。
Objective To investigate the clinicopathological features and the influencing factors of prognosis in the elderly patients with breast cancer.Methods A total of 217 elderly female patients with breast cancer treated in our hospital from March 2010 to March 2013 were collected.According to the age,the patients were divided into 65-75 years old group(n=145)and>75 years old group(n=72).The clinical and pathological features of the two groups were observed,and univariate analysis and multivariate analysis were used to analyze the influencing factors of the 5-year overall survival time(OS)and disease-free survival time(DFS).Results There were no significant differences in family history of breast cancer,histological features,estrogen receptor(ER)and progesterone receptor(PR)between the two groups(P>0.05).There were significant differences in the comorbidities,tumor size,lymph node metastasis,pathological types,clinical stages and human epidermal growth factor receptor-2(HER-2)between the two groups(P<0.05).The multivariate analysis showed that the influencing factors of OS within 5 years in the elderly patients with breast cancer were age,lymph node metastasis and PR(P<0.05).And the influencing factors of DFS within 5 years in the elderly patients with breast cancer were age and lymph node metastasis(P<0.05).Conclusions The clinicopathological features of breast cancerare are different in different age groups.The main factors affecting prognosis are age,lymph node metastasis,PR and histological features.
