摘要
目的探讨医用胶原蛋白海绵在经皮椎间孔镜技术治疗腰椎管狭窄症中的临床疗效。方法选取2017年8月至2018年8月行经皮椎间孔镜技术治疗腰椎管狭窄症的66例患者,随机分为治疗组医用胶原蛋白海绵合并传统方式止血34例,对照组传统方式止血32例。统计2组手术时间、术中出血量、术后引流量、并发症及术后住院时间,术前1 d、术后1、3、6个月和术后1年疼痛视觉模拟评分(VAS)及Oswestry功能障碍指数(ODI),按照MacNab腰椎功能标准评价2组临床疗效。结果2组手术时间、术中出血量、术后引流量及术后1个月VAS评分比较,差异有统计学意义(P>0.05);2组住院时间、并发症及术后3个月以后VAS及ODI评分无统计学意义(P>0.05)。术后1年治疗组优良率85.39%,对照组优良率为84.37%,2组优良率比较差异无统计学意义(P<0.05)。结论在经皮椎间孔镜技术治疗腰椎管狭窄症术中应用医用胶原蛋白海绵,可减少术中出血,降低手术时间,减少出血相关并发症,值得推广。
Objective To investigate the clinical efficiency of medical collagen sponge in treatment of lumbar spinal stenosis by percutaneous transforaminal endoscopic technique.Methods A total of 66 patients with lumbar spinal stenosis who were treated by percutaneous transforaminal endoscopy in our hospital from August 2017 to August 2018 were randomly divided into treatment group(n=34)control group(n=32).The patients in treatment group were treated by collagen sponge combined with traditional hemostasis,however,the patients in control group were treated by traditional hemostasis only.The operation time,intraoperative bleeding volume,postoperative drainage volume,complications and hospitalization time,pain visual analogue score(VAS)and Oswestry dysfunction index(ODI)at 1 day,1m,3m,6m,1 year after operation were observed and compared between the two groups.Moreover the clinical curative effects of both groups were evaluated according to MacNab lumbar vertebral function standard.Results There were significant differences in operation time,intraoperative bleeding volume,postoperative drainage volume and VAS scores at 1 month after operation between the two groups(P<0.05),however,there were no significant differences in hospitalization time,complications and VAS and ODI scores at 3 months after operation between the two groups(P>0.05).At one year after operation,the good/excellent rate in treatment group was 85.39%,which was 84.37%in control group,there was no significant difference between the two groups(P>0.05).Conclusion The application of medical collagen sponge in percutaneous transforaminal endoscopy for lumbar spinal stenosis can reduce intraoperative bleeding,operation time and bleeding-related complications,therefor,it is worth popularizing and applying in clinical practice.
