空洞型肺癌的PET-CT影像特点分析 被引量:4

摘要 目的分析空洞型肺癌的PET-CT影像特点。方法回顾性分析35例经病理证实空洞型肺癌患者的PET-CT表现,分析不同病理类型空洞型肺癌间的差异及病理类型、空洞壁厚度最大径、肿瘤最大径对最大标准摄取值(maximum standardized uptake value,SUV max)的影响。结果35例患者中,鳞癌15例,腺癌20例,鳞癌组SUV max为13.51±4.73,显著大于腺癌组(8.50±4.27),P<0.05;而鳞癌组空洞壁厚度最大径及肿瘤最大径均显著大于腺癌组(P值分别为0.001、0.043),两者之间空洞最大径差异无统计学意义(P>0.05)。在空洞壁厚度最大径分组(<0.5 cm,0.5~1.5 cm,>1.5 cm)及肿瘤最大径分组(<3 cm、3~5 cm,>5 cm)中,SUV max差异有统计学意义(F=10.735,P<0.001;F=6.568,P=0.004)。结论18 F-脱氧葡萄糖(18 F-FDG)PET-CT对空洞型肺癌有较好的诊断价值,且SUV max与病理类型、空洞壁厚度最大径以及肿瘤最大径存在一定的关联。 Objective To analyze the PET-CT features of cavitating lung cancer.Methods The PET-CT findings of 35 patients with cavitating lung cancer were analyzed retrospectively.Diagnoses of the subjects were confirmed by pathology and the pathological type was defined.It's aimed to analyze the difference between different pathological types of cavitating lung cancer and evaluate the influence of pathological type,the maximum thickness of the cavity wall and maximum diameter of tumor on maximum standardized uptake value(SUV max).Results Among them,squamous cell carcinoma were found in 15 patients while the other 20 patients had adenocarcinoma.The SUV max in the group of squamous cell carcinoma was 13.51±4.73,which was significantly larger than that of adenocarcinoma(8.50±4.27,P<0.05).Statistical differences were also found in maximum thickness of the cavity wall and maximum diameter of tumor between the two kinds of carcinomas(P=0.001 and P=0.043).However,the difference showed no statistical significance in maximum diameter of cavity(P>0.05).SUV max was significantly associated with the maximum thickness of the cavity wall and the maximum diameter of tumor(F=10.735,P<0.001;F=6.568,P=0.004).Conclusion 18 F-deoxyglucose(18 F-FDG)PET-CT has excellent diagnostic value for cavitating lung cancer,and SUV max is correlated with pathological type,the maximum thickness of the cavity wall and maximum diameter of tumor.
