关键反应训练对自闭症儿童的干预有效性——来自认知神经科学的证据 被引量:9

Effectiveness of Pivotal Response Training in Children with Autism——Evidence from Cognitive Neuroscience

摘要 关键反应训练(Pivotal Response Training,PRT)是一种基于应用行为分析、具有循证实践支持的自闭症儿童干预方法。有关PRT干预的认知神经科学研究多使用小样本的高功能自闭症儿童进行实验,PRT干预后,自闭症儿童的行为指标显著改善,大脑连通性改变,同时其前额叶皮层(尤其是左腹外侧前额叶皮层和左背外侧前额叶皮层)、梭状回、后颞上沟等脑区对生物运动的激活增加。未来的研究应当增加使用ERP、fMRI等认知神经科学技术探寻PRT有效性的生理标记物,以准确预测PRT对自闭症儿童的干预效果,推动自闭症儿童的干预向着精准医学的方向发展。 Pivotal Response Training(PRT) is an intervention method for children with autism based on the principles of applied behavioral analysis and supported by evidence-based practice.Cognitive neuroscience studies of PRT used small samples of high-functioning children with autism.After PRT intervention,behavioral indicators of autistic children were significantly improved,and brain connectivity was changed.Meanwhile,activation of brain areas such as left ventrolateral prefrontal cortex,left dorsolateral prefrontal cortex,fusiform gyrus and posterior superior temporal sulcus was increased.Future studies should increase the use of ERP,fMRI and other cognitive neuro-technologies to study the intervention effect of autism,and explore the physiological markers of PRT effectiveness,so as to accurately predict the intervention effect of PRT on autistic children and promote the intervention of autistic children toward the direction of precision medicine.
