摘要
目的评价介入手术治疗下颌骨动静脉畸形伴出血的效果。方法回顾性分析2012年1月至2018年1月在郑州大学第一附属医院接受介入治疗的7例下颌骨动静脉畸形伴出血患者临床资料。其中男3例,女4例;年龄8~13岁,平均(10.6±1.7)岁;均以不同程度出血为症状就诊。3例经动静脉途径栓塞治疗,4例单纯动脉途径栓塞治疗,栓塞材料为聚乙烯醇(PVA)颗粒和弹簧圈。术后随访9~18个月。结果介入治疗后7例出血症状均得到有效控制。无异位栓塞等严重并发症发生。随访期间均未复发出血,仅1例出现口腔感染,对症治疗后得到有效控制。结论急症介入治疗下颌骨动静脉畸形出血疗效确切,技术安全可行,可达到迅速止血和挽救生命的目的。
Objective To evaluate the effect of salvage interventional therapy for arteriovenous malformation of mandible(MAVM)with hemorrhage.Methods The clinical data of 7 pediatric patients with MAVM treated by interventional therapy in the First Affiliated Hospital of Zhengzhou University from January 2012 to January 2018 were retrospectively analyzed.There were 3 males and 4 females aged from 8 to 13 years.All patients had different degrees of bleeding.4 cases were treated by arterial embolization,3 cases by arterial and venous embolization.The embolic materials concluded coils and PVA particles.The follow-up time was 9-18 months.Results After interventional treatment,the bleeding of 7 children were effectively controlled,and no recurrence occurred during the follow-up period.One patient with oral infection recovered after symptomatic treatment.There were no serious complications.Conclusion Emergency interventional treatment of MAVM with hemorrhage has definite effect,which is safe,feasible and can achieve the purpose of rapid hemostasis to save life.
