摘要
目的研究股骨颈骨折关节囊减压对股骨头坏死的影响。方法选择2015年1月-2017年12月于信阳市罗山县人民医院就诊的股骨颈骨折患者124例,按随机数表法分为对照组和观察组,各62例。对照组采用闭合复位内固定治疗,观察组在对照组基础上给予关节囊解压术,两组患者术后均随访18个月。统计两组患者的围术期相关指标、内科相关并发症发生率、骨折愈合率及股骨头坏死发生率,采用髋关节Harris评分评估术后髋关节情况。结果观察组手术时间、术中出血量大于对照组(P<0.05);观察组Harris评分优于对照组(P<0.05);两组患者内科相关并发症发生率无显著差异(P>0.05);观察组骨折愈合率高于对照组,股骨头坏死率低于对照组(P>0.05)。结论在股骨颈骨折中采用关节囊减压可减少股骨头坏死发生率。
Objective To study the effects of joint capsule decompression of femoral neck fracture on femoral head necrosis.Methods A total of 124 patients with femoral neck fractures who were treated in the hospital from January 2015 to December 2017 were enrolled.They were divided into control group and observation group according to random number table method,62 cases in each group.The control group was given closed reduction and internal fixation.On this basis,observation group was given joint capsule decompression.Both groups were followed up for 18 months after surgery.The perioperative related indexes,incidence of medical related complications,fracture healing rate and incidence of femoral head necrosis in both groups were statistically analyzed.The conditions of postoperative hip joints were evaluated by hip Harris score.Results The operation time and intraoperative blood loss in observation group were higher than those in control group(P<0.05),and Harris score was better than that in control group(P<0.05).There was no significant difference in incidence of medical related complications between the two groups(P>0.05).The fracture healing rate of observation group was higher than that of control group,while rate of femoral head necrosis was lower than that of control group(P>0.05).Conclusion The application of joint capsule decompression in femoral neck fracture can reduce incidence of femoral head necrosis.
