摘要
目的探讨牵引助萌上颌埋伏阻生尖牙对骨性Ⅲ类错[牙合]青少年上颌窦形态的影响。方法对2016年1月至2018年12月就诊于郑州大学第一附属医院正畸科的20名骨性Ⅲ类错[牙合]青少年上颌埋伏尖牙行牵引助萌术。手术前后拍摄CBCT,利用InVivo Dental三维测量软件测量双侧上颌窦前后径、左右径、上下径及容积,比较手术前后双侧上颌窦形态变化。结果经过6~12个月,平均(7.45±1.50)个月,20例患者牵引助萌治疗均成功。术前,患者两侧上颌窦前后径差异无统计学意义(P>0.05),埋伏牙侧上颌窦左右径、上下径及容积均小于正常侧,差异有统计学意义(均P<0.05)。术后,两侧上颌窦前后径、左右径、上下径、容积均大于术前,差异有统计学意义(均P<0.05)。治疗前后,埋伏牙侧上颌窦上下径及容积增加量大于健侧,差异有统计学意义(均P<0.05)。结论上颌尖牙埋伏阻生对骨性Ⅲ类错[牙合]青少年上颌窦发育有一定影响,应尽早进行牵引治疗。
Objective To investigate the effect of traction of maxillary embedded cuspid teeth on maxillary sinus morphology in adolescents with skeletal class Ⅲ malocclusion.Methods Twenty adolescents with skeletal class Ⅲ malocclusion in the Department of Orthodontics of the First Affiliated Hospital of Zhengzhou University from January 2016 to December 2018 underwent traction therapy of maxillary embedded cuspid teeth.CBCT was taken before and after the operation.InVivo Dental 3D measurement software was used to measure anteroposterior diameter,transverse diameter,vertical diameter and volume of bilateral maxillary sinus.Morphological changes of bilateral maxillary sinus before and after surgery were compared.Results After 6-12(7.45±1.50)months,20 patients were all successfully treated.Before the surgery,there were no statistically significant differences in anteroposterior diameter between both sides of maxillary sinus(P>0.05).Transverse diameter,vertical diameter,and volume of the affected maxillary sinus were all smaller than those of the normal side,and the differences were statistically significant(all P<0.05).After operation,anteroposterior diameter,transverse diameter,vertical diameter,and volume of maxillary sinus on both sides were larger than before,and the differences were statistically significant(all P<0.05).During the treatment,vertical diameter and volume of maxillary sinus on the affected side increased more than those on the normal side,and the differences were statistically significant(both P<0.05).Conclusion Maxillary embedded cuspid teeth has certain impact on the development of maxillary sinus in adolescents with skeletal classⅢmalocclusion,and traction treatment should be carried out as soon as possible.
