摘要
目的 探讨树突状纤维黏液脂肪瘤(DFML)的临床病理学特征、诊断、鉴别诊断及预后。方法 收集西南医科大学附属医院2013-2018年诊断为DFML病例3例。分析其临床病理学特征及免疫组化表型,并复习相关文献。结果 3例中男性2例,女性1例,平均年龄37岁。肉眼观察肿瘤呈类圆形或不规则形,直径2. 5~5 cm,切面呈灰白色或灰黄色,胶冻状。镜下特征:肿瘤由较多的梭形细胞及星形细胞、胶原纤维束及成熟的脂肪组织组成。部分区域可见丛状血管,间质为黏液样背景。免疫组化显示其梭形细胞和星状细胞CD34、bcl-2及vimentin(+),Ki-67增殖指数<1%。结论 DFML为脂肪瘤特殊亚型,其镜下表现复杂,易误诊为梭形细胞肿瘤,其病理学特征及免疫表型有助诊断。
To investigate the clinicopathological characteristics,diagnosis,differential diagnosis and prognosis of dendritic fibromyxolipoma(DFML).Methods Three cases of dendritic fibromyxolipoma diagnosed from 2013 to 2018 at the Affiliated Hospital of Southwest Medical University were collected.The clinicopathological characteristics and immunohistochemical phenotype were analyzed and the literatures were reviewed.Results Two of the three cases were male and one was female and the mean age was 37 years.The tumor was nearly round or irregular in shape.The size of these tumors ranged from 2.5〜5 cm.The cut surface were gray-white or gray-yellow in color and myxoid in some areas.Microscopically,the tumor was characterized by a large number of spindle cells,stellate cells,collagen fiber bundles and mature adipose tissue.Plexiform vessels were seen in some areas.The stroma background was rich in myxoid.Immunohistochemically,the spindle cells and stellate cells showed that CD34,BCL-2 and vimentin were positive,and the Ki-67<1%.Conclusion Dendritic fibromyxolipoma is a rare special subtype of benign lipoma.Because of the complex microscopic appearance,it is easily misdiagnosed as spindle cell tumor.The microscopic pathological characteristics and immunohistochemical phenotype are helpful for the diagnosis.
