摘要
目的探讨宫腹腔镜联合修复术与宫腔镜定位电切术治疗剖宫产瘢痕憩室(PCSD)的效果。方法回顾性分析2016年1月~2018年11月南京医科大学附属常州市第二人民医院诊断为PCSD的患者42例,根据手术方式分为宫腔镜定位电切组及宫腹腔镜联合修复组,每组21例。宫腔镜定位电切组行单纯宫腔镜定位电切术,宫腹腔镜联合修复组行宫腹腔镜联合修复术。比较两组临床效果,手术时间、术中出血量、住院时间、住院费及术后妊娠情况。结果宫腔镜定位电切组手术时间、术中出血量、住院天数和住院费用均低于宫腹腔镜联合修复组,差异有统计学意义(P<0.05或P<0.01)。两组术后1个月临床效果比较,差异无统计学意义(P>0.05);宫腹腔镜联合修复组术后3、6个月总有效率高于宫腔镜定位电切组,差异有统计学意义(P<0.05)。宫腹腔镜联合修复组自然受孕情况高于宫腔镜定位电切组。结论两种手术方式治疗子宫瘢痕憩室均取得一定的临床效果,宫腹腔镜联合修复术更安全、有效,有利于患者预后,临床值得推广。
Objective To investigate the efficacy of laparoscopic repair with hysteroscopy and hysteroscopic niche re-section in the treatment of post-cesarean scar diverticulum.Methods From January 2016 to November 2018,42 cases with post-cesarean scar diverticulum in the Affiliated Changzhou NO.2 People′s Hospital of Nanjing Medical University were selected.According to the surgical methods,they were divided into the hysteroscopic niche resection group and the laparoscopic repair with hysteroscopy group,with 21 cases in each group.The hysteroscopic niche resection group was performed simple hysteroscopic niche resection,while the laparoscopic repair with hysteroscopy group was per-formed laparoscopic repair with hysteroscopy.The operation time,intraoperatve blood loss,hospital stay length and hos-pital expenses and postoperative pregnancy were compared between two groups.Results The operation time,intraoper-atve blood loss,hospital stay length,hospital expenses of the hysteroscopic niche resection group were lower than those of the laparoscopic repair with hysteroscopy group,and the differences were statistically significant(P<0.05 or P<0.01).There was no significant difference in the clinical effect at 1 month after surgery between two groups(P>0.05).The clinical effect at 3 and 6 months after surgery of the laparoscopic repair with hysteroscopy group were higher than those of the hysteroscopic niche resection group,and the differences were statistically significant(P<0.05).Postopera-tive pregnancy of the laparoscopic repair with hysteroscopy group were higher than those of the hysteroscopic niche re-section group.Conclusion Both surgical methods have achieved certain clinical effect in the treatment of post-cesarean scar diverticulum.The laparoscopic repair with hysteroscopy of PCSD is safer and more effective,has more satisfactory menstrual recovery,should widely use to treat patients with PCSD.
