摘要
目的:探讨导乐陪伴联合硬膜外阻滞全程无痛分娩的临床应用价值。方法:回顾性分析2017年1月-2018年12月在笔者所在医院分娩的165例产妇临床资料,85例采用导乐陪伴联合硬膜外阻滞全程无痛分娩产妇作为研究组,80例硬膜外阻滞全程无痛分娩产妇作为对照组。比较两组母婴结局。结果:研究组第一、二产程时间均明显短于对照组,第一、二产程时VAS评分均明显低于对照组,差异有统计学意义(P<0.05)。研究组产后24 h出血量及会阴撕裂、器械助产率、中转剖宫产率均明显优于对照组,差异有统计学意义(P<0.05)。两组新生儿1 min Apgar评分比较,差异均无统计学意义(P>0.05)。两组均未见宫内窘迫、新生儿窒息、缺氧脑病等不良事件发生。结论:导乐陪伴联合硬膜外阻滞全程无痛分娩有助于缩短产程、缓解疼痛,减少会阴撕裂、产后出血等并发症,保证母婴安全,可作为有效产时服务模式使用。
Objective:To explore the clinical value of doule accompany combined with epidural block in painless delivery.Method:The clinical data of 165 parturients delivered in our hospital from January 2017 to December 2018 were retrospectively analyzed.A total of 85 case were treated with doule accompany combined with epidural block in painless delivery as the study group,and 80 case were treated with epidural block in painless delivery as the control group.The maternal and infant outcomes of the two groups were compared.Result:The first and second labor processes in the study group were significantly shorter than those in the control group,and the VAS scores in the first and second labor processes were significantly lower than those in the control group,the differences were statistically significant(P<0.05).The amount of bleeding 24 h postpartum,perineal tear,instrument midwifery and transfer caesarean section rate of the study group were significantly better than those of the control group,the differences were statistically significant(P<0.05).There was no statistically significant difference in the 1 min Apgar scores between the two groups(P>0.05).No adverse events such as intrauterine distress,neonatal asphyxia and hypoxic encephalopathy were found in both groups.Conclusion:Doule accompany combined with epidural block in painless delivery can shorten labor process,relieve pain,reduce perineal tear,postpartum hemorrhage and other complications,and ensure maternal and infant safety,which can be used as an effective delivery service mode.
