摘要
目的研究甲状腺透明细胞癌的临床病例特点及鉴别诊断。方法纳入2015年1月—2016年1月这个时间段中入住本院的甲状腺透明细胞癌患者总计20例,查阅并记录本院内与研究选取甲状腺透明细胞癌患者相关的各项资料,综合分析其临床病例特点以及鉴别诊断方法。结果显微镜检查下甲状腺细胞癌患者肿瘤呈现为圆形和椭圆形,肿瘤直径1~5 cm以下,表现为棕红色和浅黄色,其中12例患者的肿瘤没有明显的包膜,8例患者肿瘤旁存在卫星状瘤小结,甲状腺患侧存在浸润的瘤结节。结合患者的病情开展具有针对性的手术治疗方案,手术治疗结束后1年内生存患者19例,占据总选取患者的95.00%,手术治疗结束后3年内生存患者16例,占据总选取患者的80.00%。结论甲状腺透明细胞癌患者癌细胞浆透明状改变的原因主要是细胞浆中糖原积蓄。甲状腺透明细胞癌的局部侵袭性较强,选用手术切除术可以有效抑制甲状腺透明细胞癌的局部侵袭,对于提高患者的生存质量具有重要作用。
Objective To study the clinical features and differential diagnosis of clear cell carcinoma of thyroid.Methods A total of 20 patients with clear cell carcinoma of thyroid were enrolled in our hospital from January 2015 to January 2016.Comprehensive analysis of its clinical case characteristics and differential diagnosis.Results Under microscope,the thyroid cell carcinoma patients showed round and oval shape.The diameter of the tumor was 1~5 cm,showing brownish red and light yellow,12 of which had no obvious tumor.In 8 cases,there were satellite nodules and infiltrating nodules in the thyroid gland.According to the patient's condition,19 patients survived within one year after the operation,accounting for 95.00 percent of the total patients,and 16 patients survived within three years after the operation.Occupies 80.00g of total selected patients.Conclusion The main reason for the change of cytoplasm transparency in patients with clear cell carcinoma of thyroid is the accumulation of glycogen in the cytoplasm.The local invasiveness of clear cell carcinoma of thyroid gland is strong,and surgical resection can effectively suppress the local invasion of clear cell carcinoma of thyroid gland.Invasion plays an important role in improving the quality of life of patients.
