摘要
目的分析探讨对动脉瘤性蛛网膜下腔出血伴脑血管痉挛患者采用尼莫地平注射液联合前列地尔注射液进行治疗的效果。方法选择2015年2月至2017年12月本院收治的动脉瘤性蛛网膜下腔出血伴脑血管痉挛患者88例为研究对象,随机分成观察组与对照组,各44例。对照组患者采用常规的治疗措施,同时加入前列地尔注射液进行治疗,观察组患者在对照组患者的基础上加入尼莫地平注射液进行治疗;对比两组患者的临床治疗效果,对患者血浆降钙素基因相关肽和内皮素-1水平进行测量,同时测量患者血清中超敏C反应蛋白水平、白细胞介素水平和血管内皮生长因子水平,并测量血清中肿瘤坏死因子-α水平。同时对患者采用蒙特利尔认知评估量表进行认知功能的评价。结果观察组患者治疗效果优于对照组,并且观察组患者的血管内皮因子水平和炎症因子水平有了明显的改善,改善程度明显优于对照组患者(P<0.05),观察组患者的认知功能评分也明显优于对照组患者(P<0.05)。结论对动脉瘤性蛛网膜下腔出血伴脑血管痉挛患者采用尼莫地平注射液联合前列地尔注射液进行治疗,治疗效果显著,患者的症状得到有效缓解,认知功能恢复良好,因此值得临床推广应用。
Objective To analyze the effect of nimodipine injection combined with alprostadil injection on patients with aneurysmal subarachnoid hemorrhage and cerebral vasospasm. Methods 88 patients with aneurysmal subarachnoid hemorrhage and cerebral vasospasm admitted to our hospital from February 2015 to December 2017 were enrolled in the study. They were randomly divided into the control group and the observation,with 44 cases in each group. The patients in the control group received conventional treatment and were treated with alprostadil. The patients in the observation group were treated with nimodipine injection on the basis of the control group. The clinical effects of the two groups were compared. Calcitonin gene-related peptide and endothelin-1 levels were measured, and serum levels of high-sensitivity C-reactive protein, interleukin-level, vascular endothelial growth factor and serum levels of tumor necrosis factor-α were measured. At the same time, patients were assessed for cognitive function using the Montreal Cognitive Assessment Scale. Results The therapeutic effect of the observation group was better than that of the control group, and the vascular endothelial factor level and inflammatory factor level of the observation group were significantly improved, and the improvement degree was significantly better than that of the control group(P<0.05). The functional score was also significantly better than the control group(P<0.05). Conclusion Nimodipine injection combined with alprostadil injection for the treatment of aneurysmal subarachnoid hemorrhage with cerebral vasospasm has a significant therapeutic effect. The symptoms of the patient are effectively relieved and the cognitive function is well restored.It is worthy of clinical application.
