摘要
目的探讨术后早期肠内营养(EN)与肠外营养(PN)对食管癌患者应激反应、营养状况及恢复的影响。方法选取84例食管癌手术患者随机分为EN组(42例,EN)和PN组(42例,PN)。比较两组患者术前、术后7 d的应激指标、营养指标及术后恢复情况。结果术后7 d,PN组血清促肾上腺皮质激素(ACTH)、皮质醇(Cor)水平均高于术前及EN组(P<0.05);两组血清转铁蛋白(TRF)、白蛋白(ALB)水平均升高,且EN组高于PN组(P<0.05)。EN组的术后首次排气时间及住院时间均短于PN组(P<0.05)。结论食管癌患者术后予以早期EN较PN更利于减轻患者的应激反应、改善营养状况并可促进患者的恢复。
Objective To compare the effects of early enteral nutrition(EN)and parenteral nutrition(PN)on stress response,nutritional status and recovery of esophageal cancer patients.Methods A total of 84 patients with esophageal cancer were randomly divided into EN group(42 cases,EN)and PN group(42 cases,PN).The stress indicators,nutritional indicators before operation and at 7th day after operation and postoperative recovery of the two groups were compared.Results On the 7th day after operation,the levels of adrenocorticotropic hormone(ACTH)and cortisol(Cor)in the PN group were higher than those before operation and in the EN group(P<0.05);the levels of serum transferrin(TRF)and albumin(ALB)in the two groups increased,and those in the EN group were higher than the PN group(P<0.05).The time of first exhaust and hospitalization in the EN group were shorter than those in the PN group(P<0.05).Conclusion Early EN is better than PN in alleviating stress index,improving nutritional status and promoting recovery of patients with esophageal cancer after operation.
