摘要
目的:探讨超微经皮肾镜取石术(UMP)和输尿管软镜碎石术(RIRS)治疗1~2 cm肾下盏结石的临床效果。方法:选取2018年3月至2019年8月在江苏省南京江北人民医院住院治疗的130例1~2 cm肾下盏结石患者,随机分为UMP治疗组(观察组,50例)和RIRS治疗组(对照组,80例)。比较两组结石清除效果、手术时间、术后并发症发生率及术后1 d的血清炎症指标水平。结果:观察组结石清除总有效率可达96.00%,显著高于对照组的81.25%(P<0.05);观察组手术时间显著短于对照组,术中出血量显著多于对照组,术后1 d血清超敏C反应蛋白(hs-CRP)、降钙素原(PCT)及白细胞计数(WBC)水平显著低于对照组(均P<0.05);两组术后总并发症发生率比较,差异无统计学意义(P>0.05)。结论:UMP治疗1~2 cm肾下盏结石患者疗效显著优于RIRS,能明显提高结石清除效果,缩短手术时间,减轻术后炎症反应,利于患者术后恢复。
Objective:To investigate the effects of ultra-mini percutaneous nephrolithotomy(UMP)and retrograde intrarenal surgery(RIRS)in the treatment of moderate-sized(1-2 cm)renal lower caliceal calculi.Methods:A total of 130 patients with 1-2 cm renal lower caliceal calculi in our hospital from March 2018 to August 2019 were selected,and randomly assigned to receive UMP treatment(observation group,50 cases)or RIRS treatment(control group,80 cases).The effect of stone removal,operation time,the incidence of postoperative complications and the levels of serum inflammation factors on day 1 after operation were compared.Results:The total effective rate of stone removal in the observation group was significantly higher than that in the control group(96.00%vs.81.25%)(P<0.05).The operation time in the observation group was significantly shorter than that in the control group,the intraoperative blood-loss volume was higher and the levels of serum hypersensitive C reactive protein(hs-CRP),procalcitonin(PCT)and white blood cell count(WBC)after one day of surgery were significantly lower(P<0.05).The total incidence of complications was similar in the two groups(P>0.05).Conclusion:The curative effect of UMP in the treatment of 1-2 cm renal lower caliceal calculi is significantly better than that of RIRS.It can significantly improve the stone removal effect,shorten the operation time,reduce postoperative inflammatory reaction,and promote the recovery of patients.
