剥脱性青光眼角膜内皮细胞形态学研究

摘要 目的比较剥脱性青光眼(PXG)、原发性开角型青光眼(POAG)患者及正常人角膜内皮细胞密度(ECD)、变异系数(CV)及六角型细胞比例(HE)差异。方法病例对照研究。连续性选取在我院住院治疗的PXG患者72例(72只眼)、POAG患者75例(75只眼)及性别、年龄匹配的健康体检者(CON)68例(68只眼)分别作为PXG组、POAG组及CON组。所有纳入人员均测定角膜ECD、CV及HE值,对数据进行单因素方差分析。结果 PXG组、POAG组及CON组角膜ECD分别为(2308.20±430.70)个/mm^2;(2579.65±322.91)个/mm^2及(2655.74±317.58)个/mm^2。3组间角膜ECD总体差异有统计学意义(F=15.676,P=0.000),两两比较显示,PXG组角膜ECD低于POAG组及CON组,差异有统计学意义(P=0.000,0.000),POAG组角膜ECD低于CON组,差异无统计学意义(P=0.269)。3组间CV及HE值总体差异无统计学意义(F=0.025,P=0.975;F=0.132,P=0.877)。结论 PXG眼角膜ECD低于POAG眼,在临床治疗过程中应更加注意对PXG患者角膜内皮的保护。 Objective To evaluate the differences of corneal endothelial cell density(ECD), coefficient of variation(CV) and percentage of hexagonal cells(HE) among patients with pseudoexfoliation glaucoma(PXG), primary open angle glaucoma(POAG) and healthy subjects.Methods This clinical case control study enrolled patients with PXG, POAG and age-and gender-matched healthy subjects. The PXG group included 72 cases(72 eyes), the POAG group included 75 cases(75 eyes), and the control group included 68 subjects(68 eyes). All subjects underwent corneal ECD, CV and HE measurements. Data were analyzed by one-way analysis of variance. Results The corneal ECD in PXG, POAG and control groups was 2308.20±430.70/mm^2, 2579.65±322.91/mm^2 and 2655.74±317.58/mm^2, respectively. The corneal ECD was significantly different among the three groups(F=15.676,P=0.000). The corneal ECD in the PXG group was significantly lower than that of the POAG and control groups(P=0.000, 0.000). The corneal ECD in the POAG group was not significantly different from the control group(P=0.269). There were no significant differences in CV and HE between the three groups(F=0.025,P=0.975;F=0.132,P=0.877).Conclusions The corneal ECD in patients with PXG was lower than patients with POAG, suggesting that more attention should be paid to protect the corneal endothelial cell during clinical treatment for patients with PXG.
