摘要
目的探讨甲状旁腺激素(PTH)检测在慢性肾衰竭(CRF)患者病情监测中的临床价值。方法根据内生肌酐清除率(Ccr)的不同将80例CRF患者分为肾功能代偿组(20例)、肾功能失代偿组(20例)、肾功能衰竭组(20例)和尿毒症组(20例)4组。检测患者血清PTH、肌酐(sCr)、胱抑素C(Cys C)、血钙(sCa)和血磷(sP)水平。统计分析4组间各指标水平的差异,进一步分析血清PTH水平和sCr、Cys C、sCa和sP水平的相关性。结果CRF患者血清PTH、sCr和sP水平在4组间逐步升高(P<0.05);肾功能衰竭组和尿毒症组Cys C水平高于肾功能代偿组、肾功能失代偿组(P<0.05);肾功能衰竭组和尿毒症组sCa水平低于肾功能代偿组、肾功能失代偿组(P<0.05);CRF患者血清PTH水平与sCr、Cys C、sP水平呈正相关(r=0.835,P<0.05;r=0.632,P<0.05;r=0.790,P<0.05),与sCa水平呈负相关(r=-0.325,P<0.05)。结论血清PTH水平与CRF患者病情进展密切相关,加强PTH检测有助于评估病情进展及临床疗效。
Objective To investigate the clinical value of parathyroid hormone(PTH)detection in the monitoring of patients with chronic renal failure(CRF).Methods According to the difference of endogenous creatinine clearance rate(Ccr),80 CRF patients were divided into 4 groups,with20 cases in renal function compensatory group,20 cases renal function decompensation group,20 cases in renal failure group and 20 cases in uremia group.Serum PTH,creatinine(sCr),cystatin C(Cys C),serum calcium(sCa)and serum phosphorus(sP)were detected.Statistical analysis was made in the difference of each indicator level among the 4 groups,and the correlation between serum PTH level and the levels of sCr,Cys C,sCa and sP was further analyzed.Results The levels of serum PTH,sCr and sP in patients with CRF increased gradually(P<0.05).The Cys C level of renal failure group and uremia group was higher than that of renal decompensation group and renal decompensation group(P<0.05).The sCa level of renal failure group and uremia group was lower than that of renal decompensation group and renal decompensation group(P<0.05).Serum PTH level of CRF patients was positively correlated with sCr,Cys C and sP levels(r=0.835,P<0.05;r=0.632,P<0.05;r=0.790,P<0.05),negatively correlated with sCa level(r=-0.325,P<0.05).Conclusion Serum PTH level is closely related to the disease progression of CRF patients,and strengthening PTH detection is helpful to evaluate the disease progression and clinical efficacy.
