摘要
目的探讨不同剂量来曲唑(LE)用于Ⅱ型排卵障碍患者行冻融胚胎移植(FET)准备内膜的应用效果。方法收集2013年9月至2017年11月在湘雅医院生殖医学中心要求行FET助孕的65例Ⅱ型排卵障碍患者为研究对象。按照随机原则,将患者分为LE低剂量(2.5 mg)组(n=24)、LE高剂量(5.0 mg)组(n=21)和激素替代周期组(HRT组,n=20),比较3组患者的一般资料、排卵前/黄体酮转化日外周血的雌孕激素水平、内膜厚度及分型、胚胎移植日内膜厚度,以及胚胎着床率、临床妊娠率和活产率等。结果LE高剂量组的平均成熟卵泡数显著高于LE低剂量组(P<0.05);LE高、低剂量组的移植日内膜厚度显著大于HRT组(P<0.05);LE低剂量组的排卵前子宫内膜A型比例为41.67%,显著高于HRT组(10.00%)(P<0.05)。LE低剂量组和HRT组的胚胎着床率、临床妊娠率和活产率显著高于LE高剂量组(P<0.05)。结论LE低剂量方案和HRT方案用于Ⅱ型排卵障碍患者FET周期内膜准备较LE高剂量方案能获得更好的临床结局,且LE低剂量方案的经济效益更好,更有优势。
Objective:To investigate the effect of different doses of letrozole(LE)in preparing endometrium for frozen-thawed embryo transfer(FET)cycles in patients with typeⅡovulation disorder.Methods:The 65 patients with typeⅡovulation disorder,who underwent FET in Reproductive Medicine Center of Xiangya Hospital from Sep.2013 to Nov.2017,were collected.According to the random principle,the patients were randomly assigned to LE low dose(2.5 mg)group(n=24),LE high dose(5.0 mg)group(n=21)and hormone replacement therapy(HRT)group(n=20).The general condition,estrogen and progesterone levels in peripheral blood and endometrial thickness&type on preovulation/progesterone transformation day,endometrial thickness on FET day,implantation rates,clinical pregnancy rate and live birth rate were compared among the three groups.Results:Average number of mature follicles in high dose of LE group was significantly more than that of low dose of LE group(P<0.05).The proportion of endometrial type A in low dose of LE group was 41.67%,which was significantly higher than that in HRT group(10.00%)(P<0.05).The implantation rate,clinical pregnancy rate and live birth rate of LE low dose group and HRT group were significantly higher than those of LE high dose group(P<0.05).Conclusions:In terms of FET endometrial preparation in the patients with typeⅡovulation dysfunction,LE low dose and HRT regimen can achieve better pregnancy outcomes,and the LE low dose regimen has better economic benefits and advantages.
