摘要
目的中国在老龄化进程加快的同时面临着严重的医养分离问题,老年人医疗卫生与长期照护服务需求不能同时兼顾。通过分析我国医养分离的根本原因及原因间的相互关系,为我国医养整合体系的建设提供科学依据。方法从我国医疗卫生体系与长期照护体系建设的现状出发,梳理相关政策,在明确各类服务机构服务内容和功能定位的情况下剖析医养分离问题产生的原因,推导其形成机制。结论卫生与民政体系独立且分离、服务机构功能定位不明、统一需求评估和出入院标准缺失以及长期照护服务保障的不健全等一系列问题同时存在,导致老年人选择性地涌入医疗卫生机构并长期占有床位,引发服务利用不公平和供需矛盾。
OBJECTIVE China was facing a serious separation of medical care because the aging process was accelerating,but the needs of the elderly for medical care and long-term care services could not be taken into account at the same time.It aimed to explore the reason of the separation of medical care in China and the interrelationship between the causes,and provide a scientific basis for the construction of China’s medical integration system.METHODS From the current situation of China’s medical and health system and long-term care sys tem construction,it sorted out relevant policies,analyzed the causes of separation of medical care and provided a mechanism for its for mation under the condition of clarifying the service content and function orientation of various service organizations.CONCLUSION The si multaneous and separate separation of health and civil affairs systems,the unclear function of service institutions,the lack of uniform needed assessment and the lack of standards for admission and hospitalization,and the imperfections of long-term care services had led to the selective influx of older people into health care institutions and the unfair use of services.
