摘要
目的:探讨趋化因子16(CXCL16)与血管内皮生长因子(VEGF)联合检测对良、恶性胸腔积液的诊断价值。方法:选取2017年5月至2019年5月河北省邢台市第三医院收治的98例胸腔积液患者,其中50例肺癌致胸腔积液患者为恶性组,48例肺炎致胸腔积液患者为良性组。比较两组胸腔积液及血清中CXCL16和VEGF水平,采用Pearson相关分析方法分析胸腔积液CXCL16与VEGF水平的相关性,受试者工作特征(ROC)曲线分析CXCL16和VEGF对良、恶性胸腔积液的鉴别诊断价值。结果:恶性组胸腔积液及血清中CXCL16和VEGF水平显著高于良性组(P<0.05);良性组胸腔积液及血清中CXCL16与VEGF无相关性(P>0.05),恶性组胸腔积液及血清中CXCL16与VEGF呈正相关关系(P<0.05);胸腔积液CXCL16和VEGF联合检测鉴别诊断良、恶性胸腔积液的ROC曲线下面积(AUC)为0.955(95%CI为0.907~1.000),明显高于血清CXCL16和VEGF联合检测的0.890(95%CI为0.816~0.963)。结论:胸腔积液CXCL16与VEGF联合检测对良、恶性胸腔积液有一定的鉴别诊断价值,有望成为临床恶性胸腔积液的新型分子标志物。
Objective:To investigate the value of combined detection of CXC chemokine ligand 16(CXCL16)and vascular endothelial growth factor(VEGF)in the diagnosis of benign and malignant pleural effusion.Meth ods:The clinical data of 98 pleural effusion patients including 50 lung cancer induced pleural effusion(malignant group)and 48 pneumonia-induced pleural effusion(benign group)in our hospital from May 2017 to May 2019 were collected and analyzed.The levels of CXCL16 and VEGF in pleural effusion and serum were detected,and the correlation between the CXCL16 and VEGF levels in pleural effusion was determined by Pearson correlation analysis.The differential diagnosis value of CXCL16 and VEGF for benign and malignant pleural effusion was analyzed by receiver operating characteristic(ROC)curve.Results:The levels of CXCL16 and VEGF in serum and pleural effusion of malignant group were significantly higher than those in benign group(P<0.05).The CXCL16 level was not associated with VEGF level in serum and pleural effusion of benign group(P>0.05),whereas a positive correlation was found in CXCL16 and VEGF in malignant group(P<0.05).The area under the ROC curve of combined detection of CXCL16 and VEG levels in pleural effusion was 0.955(95%CI:0.907-1.000),which was significantly higher than that of combined detection of serum levels of CXCL16 and VEGF[0.890(95%CI:0.816-0.963)](P<0.05).Conclusion:The combined detection of CXCL16 and VEGF in pleural effusion has certain value in the diagnosis of malignant pleural effusion.CXCL16 and VEGF are expected to be new markers for malignant pleural effusions.
