数字化吞咽造影录像在急性缺血性脑卒中后吞咽障碍患者中的应用 被引量:10

摘要 目的:探讨数字化吞咽造影录像在急性缺血性脑卒中后吞咽障碍患者中的影像学价值。方法:53例急性缺血性脑卒中后经吞咽造影确诊吞咽障碍的患者,分析其影像学表现。统计学方法采用Logistic回归分析。结果:急性缺血性脑卒中后,任何参与吞咽的器官均可受累,口期吞咽功能障碍者10例,咽期吞咽功能障碍者15例,口咽期合并吞咽功能障碍者28例,误吸者33例。其中,舌根软腭接触不全(P=0.048),会厌折返不全(P=0.005),环咽肌功能紊乱(P=0.022),喉闭合不全(P=0.000)与误吸有统计学意义。结论:数字化吞咽造影录像能够更加细致、全面地对急性缺血性脑卒中后吞咽障碍患者进行分期及定性分析,可用于协助精准诊治吞咽功能障碍。 Objective: To explore the image value of digital videofluoroscopic swallowing study(VFSS) in the examination of dysphagia after acute ischemic stroke. Methods: 53 acute ischemic stroke patients with dysphagia were recruited in this study. The imaging performance were gathered and analyzed. The methods were Logistic regression analysis. Results: After acute ischemic stroke, any organ involved in swallowing can be affected. There were 10 cases with dysphagia in oral stage, 15 cases with dysphagia in pharyngeal stage, 28 cases with dysphagia in oral stage and 33 cases with aspiration by mistake. Statistic analysis showed that poor tongue-soft palate contact(P=0.048), poor epiglottis retracement(P=0.005), cricopharyngeus dysfunction(P=0.022), laryngeal insufficiency(P =0.000), and were statistically significant with accidental aspiration. Conclusion: Dysphagia after acute ischemic stroke can be staged and qualitatively analyzed by digital videofluoroscopic swallowing study(VFSS)more detailed and comprehensively, which can be used to assist in the diagnosis and treatment of dysphagia.
