摘要
目的探讨护理改善专案对提高门诊胃食管反流患者自我管理能力的影响。方法采用方便抽样法,选取2017年1-12月我院门诊治疗120例胃食管反流(GERD)患者作为研究对象,按随机数字表法分为观察组60例和对照组60例。在常规门诊药物治疗基础上,对照组实施常规随访护理;观察组在对照组基础上运用护理改善专案方法分析门诊胃食管反流患者自我管理能力差的原因,制定随访管理和健康教育方案,比较干预前后两组患者临床症状、自我管理能力行为、自我效能水平及服药依从性的变化。结果干预前,两组患者各项指标比较差异无统计学意义(P>0.05)。在干预2个月后,观察组自我管理行为的认知症状管理、医护沟通得分,自我效能的疾病共性管理、自我效能总分及服药依从性优于对照组,差异有统计学意义(P<0.05),而在GerdQ、自我管理行为的运动锻炼及自我效能的症状管理得分比较差异无统计学意义(P>0.05)。随着干预时间增加,观察组干预4个月和6个月后患者GerdQ、自我管理行为、自我效能及服药依从性各维度得分均优于对照组,差异有统计学意义(P<0.05)。结论对门诊GERD患者实施长期护理改善专案,制定不同GERD病程健康教育材料,实施个性化健康教育措施,可缓解患者心理压力,能增强患者自我管理行为能力,提升自我效能水平及增加服药依从性,从而改善其临床症状。
Objective To improve self-management ability of outpatients with gastroesophageal reflux disease(GRED)by nursing project.Methods A total of 120 GRED patients were selected from the department of gastroenterology at the Tongji hospital of Wuhan.They were divided into two groups,60 cases in eachgroup.In addition to drug therapy and routine health education,the nursing project method was analyzed reasons and developed standard process of follow-up management and health education in the experimental group.This analysis compared the changes including clinical symptoms,self-management ability,self-efficacy and medication compliance were compared between the two groups.Results Except for 2 months of intervention,there was no statistical difference of the clinical symptoms between the experimental group and the control group(P>0.05)before intervenfion.At the other observation time points,the clinical symptoms,self-management ability,self-efficacy and medication compliance of the experimental group were better than those of the control group(P<0.05).Conclusion The implementation of conyterm nursing improvement project can improve in patients with out-patient gastroesophageal reflux.The application of nursing project can improve self-management behavior,self-efficacy medication compliance and relive clinical symptoms in GRED patients.
