规范化管理在四川省护理员队伍建设中的应用研究 被引量:19

摘要 [目的]探讨规范化管理在四川省护理员队伍建设中的应用效果。[方法]采用多阶段分层比例抽样法,在全省279所二级及以上公立医院中随机抽取45所医院(三级医院20所、二级医院25所)中的1 589护理员为研究对象,进行横断面调查,同时随机抽取在岗护士、病人,分别就护理员规范化管理前后护士对护理员满意度、病人对护理员满意度及护理员自测健康评定情况进行评价。[结果]护理员经规范化管理、培训,取得了考核合格证书或职业技能鉴定考试合格证书的护理员分别为1 211人(76.21%)、1 327人(83.51%);属于第三方中介公司的1 301人(81.88%),纳入医疗机构护理垂直管理体系的24人(1.51%);护士对护理员的满意度从62.04%提升到81.51%(Z=11.824,P=0.001);病人对护理员满意率为81.37%;护理员自测健康评定量表得分整体优于成都市三级甲等医院护工(t=4.989,P<0.001)。[结论]四川省护理员规范化管理体系逐步建立,但队伍结构仍需进一步优化;未来应进一步规范行业准入,加快辅助型护理人员培养。 Objective:To explore the application effect of standardized management in construction of nursing assistants in Sichuan province.Methods:Using multi stage stratified proportional sampling method,the 1589 caregivers form 45 hospitals(20 tertiary hospitals,25 secondary hospitals),as the research objects,were randomly selected in 279 secondary and above public hospitals in Sichuan province.And the cross sectional survey was carried out.At the same time,before and after the caregivers receiving standardization management,the satisfication of the nurse and the patients to caregivers,and caregivers'self test health assessment were evaluated.Results:After standardized management and training,1211 nursing assistants(76.21%)have obtained the certificate of assessment and 1327 nursing assistants(83.51%)have obtained the certificate of professional skill appraisal.1301 people(81.88%)belong to the third party intermediary company,and 24 people(1.51%)are included in the nursing vertical management system of medical institutions.The satisfaction of nurses to nursing assistants increased from 62.09%to 81.51%(Z=11.824,P=0.001).The satisfaction rate of patients to nurses was 81.37%.The total score of self rated health rating scale of nursing assistants was better than that of nursing workers in Chengdu tertiary grade A hospitals(t=4.989,P<0.001).Conclusion:The standardized management system of nursing assistants in Sichuan province has gradually established.However,the team structure has still needed to be further optimized.In the future,we should further standardize the industry access and accelerate the training of auxiliary nursing personnel.
