摘要
目的了解立体定向放射治疗(SBRT)不同剂量治疗方案对肺癌患者疗效及血清淀粉酶A(SAA)、血清第10号染色体缺失的磷酸酶和张力蛋白同源物基因(PTEN)水平的影响。方法将2015年11月至2017年4月接受放疗的141例肺癌患者随机分为高剂量组(n=71)和低剂量组(n=70)。高剂量组采用BED为90~105 Gy的放疗剂量,低剂量组应用BED为70~78 Gy的放疗剂量。对比2组临床疗效、生存、复发及转移情况、SAA、血清PTEN水平、不良反应发生率。结果高剂量组的治疗总有效率74.65%与低剂量组的65.71%比较差异无统计学意义(P>0.05);高剂量组的生存、复发及转移率分别为87.32%、8.45%、11.27%,低剂量组的则分别为82.86%、12.86%、14.29%,2组比较差异无统计学意义(P>0.05);治疗前后,2组SAA、血清PTEN水平无差异(P>0.05);高剂量组不良反应发生率21.13%高于低剂量组8.57%(P<0.05)。结论高剂量与低剂量SBRT方案对肺癌患者疗效及对SAA、血清PTEN水平的影响无明显差异,且低剂量时的不良反应少,更为安全可靠。
Objective To investigate the curative effects of different dosage of stereotaxis radiotherapy(SBRT)on lung cancer and their effects on serum amylase A(SAA),the serum levels of phosphatase and tensin congener gene(PTEN)deleted in No.10 chromosome.Methods A total of 141 patients with lung cancer who received radiotherapy in our hospital from November 2015 to April 2017 were randomly divided into high dose group(n=71)and low dose group(n=70).The patients in high dose group were treated by BED with 90~105Gy of radiotherapy dose,however the patients in low does group were treated by BED with 70~78Gy of radiotherapy dose.The clinical efficacy,survival,recurrence and metastasis,SAA,serum level of PTEN,and incidence of adverse reactions were observed and compared between the two groups.Results There was no statistically significant difference in the total effective rate between the high does group and the low does group(74.65%vs 65.71%,P>0.05).There were no statistically significant differences in survival(87.32%vs 82.86%),recurrence rate(8.45%vs 12.86%),and metastasis rate(11.27%vs 14.29%)between the high does group and the low does group(P>0.05).Moreover there were no significant differences in SAA,serum level of PTEN in both groups between before and after treatment(P>0.05).However the incidence of adverse reactions in the high dose group was significantly higher than that in the low dose group(21.13%vs 8.57%,P<0.05).Conclusion There are no significant differences in curative effects and the effects on SAA and serum levels of PTEN between high dose SBRT scheme and low dose SBRT scheme,however the low dose SBRT scheme has less adverse reactions,which is more safe and reliable.
