摘要
目的探讨卡前列素氨丁三醇联合低位B-Lynch缝合术对难治性胎盘前置性产后出血的临床疗效。方法选取2014年6月至2016年6月厦门市妇幼保健院诊治的58例难治性前置胎盘性产后出血患者作为研究对象。按处理方案划分为两组。观察组31例采用卡前列素氨丁三醇联合低位B-Lynch缝合术治疗;对照组27例采用缩宫素联合“8”字间断缝合术治疗。对比2组术中及术后出血量、临床疗效、不良反应情况。结果两组术中出血量对比,差异无统计学意义(P>0.05);观察组术后2h、术后24h内阴道出血量明显低于对照组,差异有统计学意义(P<0.05);观察组临床疗效明显优于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率明显低于对照组,差异有统计学意义(P<0.05)。结论卡前列素氨丁三醇联合低位B-Lynch缝合术治疗难治性前置胎盘行产后出血疗效确切、安全性高,值得应用。
Objective To explore the clinical curative effect of carprostol tromethamine joint low B-Lynch suture in the treatment of refractory placenta previa postpartum hemorrhage.Methods 58 patients with refractory placenta previa postpartum hemorrhage from June 2014 to June 2016 in our hospital were selected and according to the treatment scheme were divided into two groups.The 31 cases in the observation group adopted carprostol tromethamine joint low B-Lynch suture,while the 28 cases in the control group received oxytocin joint the treatment of"8"interrupted suture.Intraoperative and postoperative blood loss,clinical curative effect and adverse reaction in the two groups were compared.Results There was no statistically significant difference in intraoperative blood loss between the two groups(P>0.05).The vaginal haemorrhage amount within 2h and 24h postoperative in the observation group was significantly lower than that in the control group,with statistically significant differences(P<0.05).The clinical curative effect of observation group was obviously superior to that of control group,with statistically significant difference(P<0.05).The incidence of adverse reactions in the observation group was obviously lower than that in the control group,with statistically significant difference(P<0.05).Conclusions Carprostol tromethamine joint low B-Lynch suture is of exact curative effect and high safety in the treatment of refractory placenta previa postpartum hemorrhage,which is worthy of application.
