摘要
目的评价神经系统急危重症疾病患者应用有创颅内压(ICP)监测的价值。方法选入本院2015年1月-2018年12月收治的神经系统急危重症疾病患者268例,随机分为对照组(134例)和观察组(134例),其中对照组根据临床表现、神经系统体征、影像学资料等传统方法判定ICP,观察组采用有创ICP监测手段持续动态监测ICP。观察并记录两组患者脱水剂用量、并发症以及预后情况等指标,并进行统计比较。结果观察组脱水剂使用时间、剂量、ICU住院时长及总住院时间均显著低于对照组(P<0.05),发生继发颅内血肿、肾功能损伤、电解质紊乱及应激性溃疡的比例亦明显低于对照组(P<0.05),而预后良好率显著高于对照组(82.09%vs.51.49%,P<0.05)。结论对神经系统急危重症患者实施有创ICP监测,能及时、准确地反映患者病情,指导脱水剂的使用,缩短住院时间,减少并发症,且能更好的改善预后。
Objective To evaluate the value of invasive intracranial pressure(ICP)monitoring in critical neurological patients.Methods 268 patients with critical nervous system diseases admitted to our hospital from January 2015 to December 2018 were randomly divided into control group(134 cases)and observation group(134 cases).The control group were determined ICP according to traditional methods such as clinical manifestations,nervous system signs and imaging data,while the observation group was monitored by invasive ICP.The dosage of dehydrant,complications and prognosis were compared between the two groups.Results The using time and dose of dehydrant,length of stay in ICU and total length of stay in the observation group were significantly lower than those in the control group(P<0.05),while the incidence of secondary intracranial hematoma,renal function injury,electrolyte disturbance and stress ulcer in the observation group was also significantly lower than those in the control group(P<0.05).The prognosis rate of the observation group was significantly higher than that of the control group(82.09%vs.51.49%,P<0.05).Conclusion Invasive ICP monitoring for patients with critical nervous system diseases can timely and accurately reflect the patient's condition,guide the use of dehydrants,shorten hospital stay,reduce complications and improve prognosis.
