利用ROC曲线评价C-反应蛋白与降钙素原在血流感染中的诊断价值

Evaluation of the Diagnostic Value of C-reactive Protein and Procalcitonin in Bloodstream Infection Using ROC Curve

摘要 目的研究在血流感染中的诊断中C-反应蛋白与降钙素原检测的应用价值经ROC曲线评价的结果。方法选取2017年1—12月该院血流感染患者46例为对象,视作观察组,另外选取50例确定为非血流感染的患者为对照组,均进行血培养,并进行C-反应蛋白与降钙素原水平测定,利用ROC曲线进行评价,分析结果。结果血培养分离出病原菌46株,革兰阴性菌占65.22%,革兰阳性菌占34.78%。革兰阴性菌患者降钙素原、C反应蛋白水平均明显高于革兰阳性菌患者(t=3.625、5.827,P<0.05);血培养阴性患者的C反应蛋白及降钙素原水平均明显低于血培养阳性患者(t=5.291、7.620,P<0.05);降钙素原的AUC值大于C反应蛋白,C反应蛋白诊断血流感染的最佳阈值为61.6 mg/L,降钙素原诊断为1.35μg/L,降钙素原相应的敏感度为76.1%,特异度为92.0%,C反应蛋白为78.3%,56.0%。结论血流感染诊断中进行C-反应蛋白与降钙素原水平测定有良好价值,降钙素原指标的应用价值更为突出。 Objective To study the application value of C-reactive protein and procalcitonin in the diagnosis of bloodstream infection by ROC curve.Methods A total of 46 patients with bloodstream infections from January to December 2017 were enrolled in the observation group.Another 50 patients with non-bloodstream infections were selected as the control group.C-reactive protein and procalcitonin levels were measured,evaluated by ROC curve,and analyzed.Results 46 strains of pathogenic bacteria were isolated from blood culture,Gram-negative bacteria accounted for 65.22%,and Gram-positive bacteria accounted for 34.78%.The levels of procalcitonin and C-reactive protein in Gram-negative bacteria were significantly higher than those in Gram-positive bacteria(t=3.625,5.827,P<0.05).The levels of C-reactive protein and procalcitonin in patients with negative blood culture were significantly higher.Lower than blood culture positive patients(t=5.291,7.620,P<0.05);the AUC value of procalcitonin was greater than C-reactive protein,and the optimal threshold for C-reactive protein diagnosis of bloodstream infection was 61.6 mg/L.Calcitonin original diagnosis was 1.35μg/L,the corresponding sensitivity of procalcitonin was 76.1%,the specificity was 92.0%,and the C-reactive protein was 78.3%,56.0%.Conclusion The diagnosis of bloodstream infection has good value in the determination of C-reactive protein and procalcitonin level.The application value of procalcitonin index is more prominent.
