摘要
目的分析52例直肠神经内分泌肿瘤(NEN)患者内镜超声(EUS)征象并探讨其临床诊断价值。方法我院消化内科收治的52例直肠NEN患者(观察组)及同期就诊的50例直肠其它黏膜病变患者(对照组),分析观察组临床表现及两组EUS声像特点。结果直肠NEN肿瘤EUS诊断:敏感性94.23%(49/52),特异性88.00%(44/50),阳性预测值92.45%(49/55),阴性预测值93.62%(44/47),准确度89.61%,约登指数0.77。直肠NEN与直肠其它黏膜病灶区的灰度值差异有统计学意义(P<0.01)。结论通过EUS对直肠NEN与直肠其它黏膜病变的诊断,两者声像特点及病理结构差异较大,EUS对直肠NEN的诊断,以及直肠其他黏膜下病变的鉴别具有较显著的临床意义。
Objective To analyze the endoscopic ultrasonography(EUS) signs and clinical diagnostic value of 52 patients with rectal neuroendocrine neoplasms(NEN).Methods A retrospective analysis of 52 rectal NEN patients(observed group) admitted to our hospital and 50 patients with rectal other mucosal lesions in the same period(control group) was performed.The clinical manifestations of the patients in the observation group and the EUS images of the two groups were analyzed.Results EUS diagnosis of rectal NEN showed that the sensitivity was 94.23%(49/52),the specificity was 88.00%(44/50),the positive predictive value was 92.45%(49/55),the negative predictive value was 93.62%(44/47),accuracy was 89.61% and the Yoden index was 0.77.There was a significant difference in the gray value between rectal NEN and other mucosal lesions in the rectum(P <0.01).Conclusion The diagnosis of rectal NEN and other mucosal lesions of the rectum by EUS has There are different imaging characteristics and pathological structure between rectal NEN and other mucosal lesions of the rectum through EUS.EUS has a significant clinical value for diagnosis of rectal NEN and differentiation diagnosis of other mucosal lesions of the rectum.
