摘要
目的探讨3.0T磁共振多参数成像(mp-MRI)对前列腺癌诊断效能的临床价值。方法选取2016年6月~2018年6月于珠海市中西医结合医院经穿刺病理证实的82例前列腺疾病患者(前列腺癌组38例,前列腺良性病变组44例),采用T 2WI、DWI、^1H-MRS及DCE-MRI扫描,对各序列图像及参数进行分析、测量,以ROC曲线下面积比较不同序列的诊断效能。结果1)38例前列腺癌,44例前列腺良性病变(前列腺增生23例、增生伴慢性炎21例);2)前列腺癌组和前列腺良性病变组的各参数值比较均有显著差异(P<0.05);3)各序列ROC曲线分析显示,对前列腺癌的诊断效能为DWI(AUC=0.96)、DCE-MR(AUC=0.93)、1H-MRS(AUC=0.87)、T 2WI(AUC=0.77);4)3.0T mp-MRI前列腺癌多参数成像联合诊断前列腺癌的敏感度、特异度和准确度分别为94.74%、86.36%、90.24%。结论3.0T mp-MRI对前列腺癌诊断效能由高到低依次为DWI、DCE-MRI、^1H-MRS、T2WI。mp-MRI联合诊断能提高前列腺癌准确性。
Objective This study is to investigate the clinical value of 3.0 T multi-parameter magnetic resonance(mp-MRI)in the diagnosis of prostate cancer.Methods We selected 82 patients with prostate disease confirmed by pathology in zhuhai combine traditional chinese and western medicine hospital in June 2016 to June 2018.These patients were subjected to the conventional sequences(T1WI and T2WI),DWI,^1H-MRS,and DCE-MRI scanning,and the image data and parameters were analzyed and compared.ROC analysis was performed to assess the therapeutic efficacies.Results According to the puncture and biopsy,in these 82 patients,there were 38 cases of prostate cancer and 44 cases of benign prostatic lesions(23 cases of benign prostatic hyperplasia and 21 cases of benign prostatic hyperplasia accompanied with chronic inflammation).Our results showed that significant differences were observed in the imaging parameters between the prostate cancer and benign prostatic lesion groups.ROC analysis showed that the diagnostic efficacies exhibited a declining trend in the following order:DWI(AUC=0.96),DCE-MRI(AUC=0.93),^1H-MRS(AUC=0.87),and T2WI(AUC=0.77).Moreover,the diagnostic accuracy of mp-MRI was 90.24%,the sensitivity was 94.74%,and the specificity was 86.36%,respectively.Conclusion 3.0 T mp-MRI combination detection significantly improves the diagnostic accuracy of prostate cancer,which represents a non-invasive and effective imaging diagnostic method for prostate cancer.
