摘要
目的观察腹腔镜下胃楔形切除在胃间质瘤(GIST)治疗中的效果及其安全性,为GIST手术治疗的优化提供帮助。方法将符合入选标准的胃间质瘤患者62例按贯序法分为A、B两组,A组(n=31)接受常规传统开腹术式治疗,B组(n=31)则采用腹腔镜下胃楔形切除术。观察比较两组患者的切口长度、手术时间、排气时间、失血总量、住院天数等术中、术后情况;加强术后规范随访,对两组术后并发症情况及相关生存质量指标进行评价。结果与采用开腹术式的A组比较,采用腹腔镜下胃楔形切除的B组在手术切口长度、术中出血量、术后排气时间、术后卧床时间及住院天数等指标方面明显降低(P<0.05),术后并发症发生率也有明显降低(P<0.05),KPS指数评分显示,B组术后6、12个月时KPS评分较A组有所升高,体能状况的改善略优于A组,但两组比较差异无统计学意义。结论与传统开腹术式相比,腹腔镜下胃楔形切除治疗具有创伤小、恢复快、疗效确切、安全性高等优势,可以成为临床治疗胃间质瘤的首选术式。
Objective To observe the effect and safety of laparoscopic gastric cuneiform resection in the treatment of gastric stroma tumor(GIST),and to provide help for the optimization of GIST operation.Methods 62 patients with gastric stromal tumor according to the inclusion criteria were divided into group A and group B according to the sequential method.Group A(n=31)was treated with conventional laparotomy,and group B(n=31)was used for laparoscopic gastric wedge resection.The length of incision,operation time,time of exhaust,total amount of blood loss and number of days of hospitalization were compared in the two groups.The postoperative complications and related quality of life of the two groups were evaluated.Results Compared with group A,the length of incision,the amount of bleeding during operation,the time of postoperative exhaust,the time of bed rest and the days of hospitalization in group B were significantly decreased(P<0.05),and the length of incision,the amount of bleeding during operation,the time of exhaust after operation,the time of stay in bed and the days of hospitalization were significantly decreased in group B(P<0.05).The incidence of postoperative complications was also significantly decreased(P<0.05).The KPS score of group B was higher than that of group A at 6 months and 12 months after operation,which showed that the improvement of physical fitness in group B was slightly better than that in group A,however,there was no significant difference between the two groups.Conclusion Compared with the traditional laparotomy,laparoscopic gastric cuneiform resection has the obvious advantages of less trauma and faster recovery,and has the advantages of accurate curative effect and high safety.It can be used as the first choice for the clinical treatment of gastric stroma tumor.
