多模态磁共振小肠造影对小肠良恶性肿瘤的诊断价值分析 被引量:4

摘要 目的分析多模态磁共振小肠造影对小肠良恶性肿瘤的诊断价值。方法回顾性选取溧阳市人民医院2016年1月至2018年12月150例疑诊小肠肿瘤性疾病的患者,均行多模态磁共振与CT小肠造影,分析患者病变部位、肠壁形态及病变累及范围等情况,并与手术结果和内镜活检病理结果进行对照分析,分别计算CT、磁共振小肠造影诊断小肠良恶性肿瘤的敏感度、特异性及准确度。结果经手术与内镜活检病理结果显示,83例患者出现小肠肿瘤性病灶(88处),其中脂肪瘤5处,海绵状血管瘤7处,神经内分泌肿瘤9处,间质瘤41处,淋巴瘤18处,腺癌8处。CT、磁共振对小肠肿瘤性疾病诊断敏感度分别为90.36%(75/83)、97.59%(81/83),特异度分别为100.00%(67/67)、100.00%(67/67),准确度分别为94.67%(142/150)、98.67%(148/150),两种影像学方法的诊断效能无显著差异(P>0.05)。结论多模态磁共振小肠造影对小肠良恶性肿瘤的诊断效能与CT检查相当,二者均具有较高的诊断价值,均可作为临床诊断小肠良恶性肿瘤的重要影像学方法。 Objective To analyze the diagnostic value of multi-modal magnetic resonance enterography in benign and malignant small intestinal tumors.Methods 150 patients with suspected small intestinal neoplasms in our hospital from January 2016 to December 2018 were selected.All patients underwent multi-modal magnetic resonance and CT enterography.The lesion location,intestinal wall morphology and the extent of involvement were analyzed.The sensitivity of CT and magnetic resonance enterography in the diagnosis of benign and malignant small intestinal tumors was calculated and compared with the results of surgery and endoscopic biopsy.Degree,specificity and accuracy.Results The pathological results of surgical and endoscopic biopsy showed that 83 patients showed small intestinal neoplasms(88 focus),including 5 lipomas,7 cavernous hemangiomas,9 neuroendocrine tumors,41 stromal tumors,18 lymphomas and 8 adenocarcinomas.The diagnostic sensitivity of CT and MR was 90.36%(75/83),97.59%(81/83),specificity was 100.00%(67/67)and 100.00%(67/67),accuracy was 94.67%(142/150)and 98.67%(148/150),respectively.There was no significant difference between the two imaging methods in diagnostic efficacy(P>0.05).Conclusion Multimodal magnetic resonance enterography is as effective as CT in the diagnosis of benign and malignant small intestinal tumors.Both of them have high diagnostic value.Both of them can be used as important imaging methods in clinical diagnosis of benign and malignant small intestinal tumors.
