摘要
目的:观察前房注气术治疗白内障超声乳化术后角膜后弹力层脱离(DMD)的临床效果。方法:收集白内障超声乳化术后发生角膜后弹力层脱离的患者28例(28眼)。所有患者均于术后3周内行前房注入消毒空气,利用气泡顶压使脱离的后弹力层复位;后弹力层复位不良或空气吸收后仍未复位者,以同样方法再次行前房注气,对后弹力层复位及视力预后情况进行临床观察。结果:28例患者经前房注气后角膜后弹力层均复位,其中17例经1次前房注气,11例经2~3次前房注气后角膜透明,视力都得到了不同程度恢复。1例重度脱离伴部分后弹力层脱失患者恢复较慢,3个月后角膜恢复透明。结论:前房注气术是治疗白内障超声乳化术后角膜后弹力层脱离的有效方法,操作可重复,创伤小,临床疗效确切。
Objective:To observe clinical efficiency of anterior air injection in treating descemet’s membrane detachment(DMD)caused by cataract phacoemulsification surgery.Methods:Twenty-eight patients(17 eyes)who were performed anterior air infusion for descemet’s membrane detachment caused by phacoemulsification cataract surgery.All patients’anterior chamber was injected with sterile air to reset DMD within 3 weeks.Repeated anterior air injection were performed when DMD had not been reset.Final status of descemet’s membrane,and best-corrected visual acuity were noted.Results:28 patients had attached descemet’s membrane after anterior air injections,and the cornea became transparent in 17 patients after a single gas injection and 7 patients after two or three gas injections.The visions of 28 patients were improved to a distinct degree.One patient’s corneal descemet’s membrane recovered slowly until 3 months after the injection.Conclusion:Anterior air injection can treat corneal Descemet’s membrane detachment during cataract surgery.The method is simple and can be operated repeatedly with obvious clinical efficiency.
