实时三维子宫输卵管超声造影与X碘油造影在输卵管源性不孕症中的一致性研究 被引量:17

A study on the consistency of transvaginal 3D ultrasound contrast hysterosalpingography versus X-ray hysterosalpingography in tubal infertility

摘要 目的探讨经阴道实时三维子宫输卵管超声造影(RT-3D-HyCoSy)在输卵管源性不孕症中的临床应用价值及与X碘油造影评估输卵管通畅性的一致性.方法选取2017年8月至2018年7月在桂林医学院附属医院考虑为输卵管源性不孕症患者18例为研究对象,患者分别进行实时三维子宫输卵管超声造影(RT-3D-HyCoSy)及X线下碘油造影(HSG),对比观察两组输卵管走形及盆腔造影剂分布情况,比较RT-3D-HyCoSy、HSG对输卵管通畅性的诊断价值的一致性.结果 18例共36条输卵管,RT-3D-HyCoSy显示输卵管通畅21条(58.3%),输卵管阻塞10条(27.8%),输卵管通而不畅5条(13.9%),HSG显示输卵管通畅21条(58.3%),输卵管阻塞9条(25.0%),输卵管通而不畅6条(16.7%),显示两种检查方法检查结果具有较好的一致性(Kappa=0.76),且RT-3D-HyCoSy可清晰显示宫腔病变3例,其中,子宫内膜息肉2例,宫腔粘连1例.结论 RT-3D-HyCoSy具有可动态立体显示子宫输卵管情况、无辐射、无碘过敏等优点,在诊断输卵管源性不孕症中操作便捷、安全、准确,值得临床推广. Objective To investigate the clinical application of transvaginal real time three-dimensional hysterosalpingo-contrast sonography (RT-3D-HyCoSy) in tubal infertility. Methods A total of 18 tubal infertile patients were recruited in the Affiliated Hospital of Guilin Medical University from Aug.2017 to Jul.2018. The patients were examination by RT-3D-HyCoSy and HSG,detection the contrast agent flowing in fallopian tubes and pelvic cavity, two methods of evaluating tubal patency were compared. Results RT-3D-HyCoSy examination showed that the patency was 21 Cases (58.3%), and obstruction was 10 Cases (27.8%), partly obstruction was 5 Cases (13.9%). HSG examination showed that the patency was 21 Cases (58.3%), and obstruction was 9 Cases (25.0%), partly obstruction was 6 Cases (16.7%). Results showing RT-3D-HyCoSy and HSG examination showed a good consistency. And RT-3D-HyCoSy showed 2 cases with intrauterine polyps, 1 case with intrauterine adhesions. Conclusion RT- 3D-HyCoSy has the advantage of real-time、dynamic,with no risk of allergic reactions to the contrast medium and with no patient exposure to radiationis, which make it a good way for evaluating fallopian tubal patency. And it is worthy of application in clinic work.
