3.0 T MR T1 ρ及T2 mapping在颈椎间盘退行性变中的应用研究 被引量:16

The application value of 3.0 T T1ρ and T2 mapping imaging in the study of cervical disc degeneration in patients

摘要 通过对56例患者(A非症状组27例与B症状组29例)采用颈椎MRI T2WI、T1ρ及T2 mapping序列检查,探讨3.0 T MR T1ρ及T2 mapping在颈椎间盘退行性变中的应用价值。结果显示56例受检者280个椎间盘Pfirrmann Ⅱ、Ⅲ级(退变早期)占比最高(73.57%)。T1ρ及T2 mapping序列可敏感观测椎间盘退变程度(P均<0.01)。T1ρ值在前纤维环(AAF)、髓核(NP)及后纤维环(PAF)区均与Pfirrmann分级间呈明显负相关(r值分别为-0.648、-0.736、-0.738,P均<0.01);T2 mapping值在AAF、NP、PAF区均与Pfirrmann分级间呈弱负相关(r值分别为-0.299、-0.450、-0.352,P均<0.01)。随椎间盘退变程度的加重T1ρ及T2 mapping值呈下降趋势,各期间差异有统计学意义(P均<0.01),T1ρ值能明显区分椎间盘退变各期(P均<0.01);T2 mapping值在区分正常与退变早期椎间盘时无统计学差异(P=0.334)。Pfirrmann分级与年龄呈正相关(r=0.683,P<0.01)。T1ρ值、T2 mapping值均与年龄呈低度负相关(r分别为0.492、-0.548,P均<0.01)。T1ρ、T2 mapping序列都能敏感鉴别症状性退变(P值均<0.01)。非症状组的T1ρ及T2 mapping值均较症状组高。T1ρ值在C2-3~C5-6髓核区域曲线下面积较同一节段的T2 mapping小,C6-7 T1ρ较T2 mapping值大。MRI测量T1ρ及T2 mapping值可量化评估颈椎间盘退变及其与年龄、不同节段的相关变化,具有一定临床价值,可作为评估早期颈椎间盘退变的重要补充序列。
