负压封闭引流联合重组人粒细胞巨噬细胞刺激因子治疗深Ⅱ度烧伤的效果及对血清ICAM-1、EPO水平的影响 被引量:5

摘要 目的研究负压封闭引流(VSD)联合重组人粒细胞巨噬细胞刺激因子(rhGM-CSF)治疗深Ⅱ度烧伤的效果及对血清细胞间粘附因子-1(ICAM-1)、促红细胞生成素(EPO)水平的影响。方法将我院收治的100例Ⅱ度烧伤患者按照随机数字表法分为对照组和观察组,每组50例。对照组采用VSD治疗,观察组采用VSD联合rhGM-CSF治疗。比较两组患者的治疗效果。结果观察组患者治疗14d后的创面愈合率显著高于对照组,创面完全愈合时间显著短于对照组(P<0.05)。治疗7d后,两组的PT及APTT显著延长,且观察组长于对照组(P<0.05)。治疗21d后,观察组创面瘢痕的色泽、厚度、柔软度、血管分布评分均显著低于对照组(P<0.05)。治疗14d后,两组的血清ICAM-1水平显著降低,血清EPO水平显著升高,且观察组均显著优于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论VSD联合rhGM-CSF治疗深Ⅱ度烧伤创面的效果显著,可有效降低血清ICAM-1水平,提高EPO水平,改善血凝功能,进而促进创面愈合。 Objective To study the effect of vacuum sealing drainage(VSD)combined with recombinant human granulocyte-macrophage colony stimulating factor(rhGM-CSF)in the treatment of deep second degree burn and its impact on serum intercellular adhesion factor-1(ICAM-1)and erythropoietin(EPO)levels.Methods One hundred cases of second degree burn patients admitted in our hospital were divided into control group and observation group by random number table method,with 50 cases in each group.The control group was treated with VSD,while the observation group was treated with VSD combined with rhGM-CSF.The therapeutic effects of the two groups were compared.Results The wound healing rate after 14 days of treatment of the observation group was significantly higher than that of the control group,and the complete healing time of the wound was significantly shorter than that of the control group(P<0.05).After 7 days of treatment,the PT and APTT of the two groups were significantly longer than those before treatment,and those of the observation group were longer than the control group(P<0.05).After 21 days of treatment,the scores of color,thickness,softness and vascular distribution of scars in the observation group were significantly lower than those in the control group(P<0.05).After 14 days of treatment,the levels of serum ICAM-1 in the two groups decreased significantly,and the levels of serum EPO increased significantly,and those of the observation group were significantly better than the control group(P<0.05).There was no significant difference in the total incidences of adverse reactions between the two groups(P>0.05).Conclusion VSD combined with rhGM-CSF is effective in treating deep second degree burn wounds.It can effectively reduce the level of serum ICAM-1,increase the level of EPO,improve the function of blood coagulation,and promote wound healing.
