摘要
目的探讨血糖信息化管理系统与传统血糖监测在重型颅脑损伤患者应激性高血糖治疗中的应用效果。方法选取2017年1月~2019年1月我院按照格拉斯哥昏迷(GCS)评分诊断重型颅脑损伤同时伴应激性高血糖92例患者,随机分为观察组和对照组,每组各46例。观察组采用血糖信息化管理系统,对照组采用传统1~3次/d血糖检测,两组均进行静脉胰岛素治疗,血糖控制目标设定为7.8~10.0mmol/L。观察两组的胰岛素治疗时间、感染率、低血糖发生率、死亡率、格拉斯哥预后(GOS)评分。结果观察组感染率、低血糖发生率、死亡率、高血糖发生率、空腹血糖、餐后2h血糖均低于对照组,GOS评分高于对照组,差异有统计学意义(P<0.05);两组GCS评分及胰岛素治疗时间比较,差异无统计学意义(P>0.05)。结论将血糖信息化管理系统应用于重型颅脑损伤伴应激性高血糖患者的治疗,不仅创新了血糖控制模式,有效地改善了重型颅脑损伤患者的预后,也为进一步优化重型颅脑损伤应激性高血糖的治疗提供了全新的模式。
Objective To explore the application effect of diabetes information management system and traditional blood glucose monitoring on stress hyperglycemia in patients with severe traumatic brain injury.Methods From January 2017 to January 2019,92 patients in our hospital were selected with severe traumatic brain injury and stress hyperglycemia which according with Glasgow coma (GCS),the research subjects were randomly divided into the observation group (n=46) and the control group (n=46).The observation group was conducted by diabetes information management system and the control group was treated with traditional blood sugar detection 1-3 times a day.Both groups received intravenous insulin therapy and the target of blood sugar control was set at 7.8-10.0 mmol/L.Insulin treatment time,infection rate,the incidence of hypoglycemia,mortality,Glasgow outcome scale (GOS) were observed between two groups.Results In the observation group,the infection rate,the incidence of hypoglycemia,the mortality rate,incidence rate of hyperglycemia,fasting blood sugar,2 h postprandial blood gucose were lower than that in the conrol group (P<0.05),and the GOS score were lower than those in the control group,and the GOS score was higher than that in the control group,the differences were statistically significant (P<0.05).There were no significant differences in GCS score and insulin treatment time between the two groups(P>0.05).Conclusion The applied the diabetes information management system to the treatment of patients with stress hyperglycemia and severe traumatic brain injury not only innovates the blood sugar control mode,traumatic brain injury is effectively improved for the prognosis of patients with severe type,but also provides a new mode for further optimizing the treatment of stress hyperglycemia in severe traumatic brain injury.
