摘要
目的分析腹腔镜下结直肠癌根治术后吻合口瘘危险因素。方法回顾性分析收治的结直肠癌根治术患者58例,对发生和未发生吻合口瘘患者的临床资料进行对比分析。结果58例患者发生术后吻合口瘘的患者5例,发生率为8.62%。发生吻合口瘘患者与未发生组在年龄、体重指数BMI、糖尿病史、术前新辅助化疗、术前肠梗阻、肿瘤距离肛门缘距离及血浆白蛋白水平方面存在显著差异(P<0.05),在性别、手术时间方面比较无显著差异(P>0.05);年龄、体重指数BMI、糖尿病史、术前新辅助化疗、术前肠梗阻、肿瘤距离肛门缘距离及血浆白蛋白水平,可作为独立危险因素存在。结论腹腔镜下结直肠癌根治术后吻合口瘘属于一种常见的并发症,疾病受到很多因素的影响,一定要全面加强危险因子的筛查,重点加强高危因子的干预管理,降低术后吻合口瘘发生率,促进患者早日康复。
Objective To analyze the risk factors of anastomotic leakage after laparoscopic radical resection of colorectal cancer.Methods 58 patients with colorectal cancer undergoing radical resection were analyzed and compared with those without anastomotic leakage.Results Postoperative anastomotic leakage occurred in 5 cases(8.62%). There were significant differences in age,body weight BMI,diabetes history,preoperative chemotherapy,preoperative intestinal obstruction,tumor anal margin distance and plasma albumin between the patients with anastomotic leakage and those without anastomotic leakage,but there was no significant difference in sex and operation time between the two groups,but there was no significant difference in age and body mass index(BMI,)diabetes history between the two groups.Preoperative neoadjuvant chemotherapy,preoperative intestinal obstruction,tumor anal margin distance and albumin can be used as independent risk factors.Conclusion Anastomotic leakage after laparoscopic radical resection of colorectal cancer is affected by many factors.It is necessary to strengthen the screening of risk factors,strengthen intervention management,reduce anastomotic leakage and promote rehabilitation.
