摘要
[目的]介绍Ilizarov外固定治疗多发性骨软骨瘤致前臂畸形的手术技术。[方法]2014年7月~2017年7月收治6例多发性骨软骨瘤前臂畸形患者,男2例,女4例,年龄6~20岁,平均(12.72±5.23)岁;应用Ilizarov外固定器行尺骨牵伸延长、桡骨截骨矫形等处理,1例患者同期行尺骨远端骨软骨瘤切除术,1例患者同期行桡骨近端骨软骨瘤切除术。[结果]患者前臂畸形矫正满意;尺骨近端延长25~35mm,平均(30.00±5.00)mm,均达到骨性愈合,未见延迟愈合;桡骨截骨矫形良好、截骨端骨性愈合;腕关节尺偏畸形得到大部分纠正,下尺桡关节复位,前臂旋转功能明显恢复,腕关节及肘关节活动功能明显改善。术后1例患者出现轻度屈腕畸形,予以配置辅具与前臂外固定器相结合很快纠正并有效预防复发。[结论]Ilizarov外固定技术治疗多发性骨软骨瘤致前臂畸形的临床效果满意,具有创伤小、并发症少、操作简便灵活等优点。
[Objective]To introduce the Ilizarov external fixation technique for forearm deformity secondary to multiple osteochondromatosis.[Methods]From July 2014 to July 2017,6 patients underwent surgical treatment for forearm deformity secondary to multiple osteochondroma in our hospital,including 2 males and 4 females aged 6~20 years.Ilizarov external fixator was used for ulnar distraction and radial correction after osteotomy.Of them,one patient received ulnar distal osteochondromatosis resection simultaneously,and another patient received proximal radius osteochondroma resection simultaneously.[Results]All the patients had the forearm deformities corrected satisfactorily,associated with the proximal ulnar lengthened 25~35 mm,which all achieved bony healing without delayed healing.In addition,the radial deformity was also corrected with bony healing to return to normal alignment after the osteotomy.The ulnar deviation of the wrist joint was mostly corrected,and the distal radioulnar joint was reduced with satisfactory restoration of the forearm rotation function and considerable improvement of the wrist and elbow function.Of them,1 patient presented mild wrist flexion contracture during correction,which was quickly corrected and effectively prevented from recurrence by installing an auxiliary device for the external fixator of the forearm.[Conclusion]This Ilizarov external fixation technique is a reliable treatment for forearm deformities secondary to multiple osteochondromatosis,with satisfactory clinical outcomes,as well as the advantages of less trauma,fewer complications,and easily flexible operation.
