摘要
目的分析胎儿脐带绕颈的临床诊断中采取产前彩超检查的临床应用价值。方法抽取院内自2017年1月-2018年12月止诊治的140例脐带绕颈孕产妇纳入观察组,另选择同期未发生脐带绕颈的正常分娩孕产妇140例纳入对照组。两组孕产妇均于产前两周接受产前彩超检查。将产前彩超结果和孕产妇的分娩结局进行对比,分析产前彩超的诊断准确率。结果经产妇分娩结局证实观察组产妇中存在胎儿脐带绕颈者134例,产前彩超的诊断准确率为95.71%;对照组产妇中经分娩证实无脐带绕颈者136例,诊断准确率为97.14%。两组对比并无显著差异(P>0.05);发生脐带绕颈138例孕产妇中的新生儿窒息、胎儿宫内窘迫以及剖宫产率均高于未发生脐带绕颈孕产妇(P<0.05)。结论产前彩超在胎儿脐带绕颈诊断中具有较高应用价值,可帮助及早发现孕产妇的脐带绕颈情况,同时胎儿脐带绕颈与分娩结局存在相关性,通过加强产前彩超检查做好分娩方式选择以及产前准备具有重要临床价值。
Objective:To analyze the clinical value of prenatal color Doppler ultrasonography in the clinical diagnosis of fetal umbilical cord around neck. Methods: 140 pregnant women who were diagnosed and treated from January 2017 to December 2018 were selected to be included in the observation group, and 140 normal parturients who had not taken place in the same period were included in the control group. The pregnant women in both groups were examined by prenatal ultrasound two weeks before delivery. The results of prenatal color Doppler were compared with the delivery outcomes of pregnant women, and the diagnostic accuracy of prenatal color Doppler was analyzed. Results:134 cases of fetal umbilical cord encircling neck were confirmed by parturient outcome in the observation group. The diagnostic accuracy of color Doppler ultrasound was 95.71%, while that of the control group was 97.14%. There was no significant difference between the two groups(P>0.05), and the neonatal asphyxia, fetal distress and cesarean section rate in 138 pregnant women with umbilical cord around neck were higher than those without umbilical cord around neck(P<0.05). Conclusion:Prenatal color Doppler ultrasonography has high application value in the diagnosis of fetal umbilical cord around neck. It can help to detect the situation of umbilical cord around neck in pregnant women as early as possible. At the same time, there is a correlation between fetal umbilical cord around neck and the outcome of delivery, so we can do a good job by strengthening prenatal color Doppler examination. The choice of delivery mode and prenatal preparation have important clinical value.
