摘要
目的观察妊娠期凝血功能的检测在预防并发静脉血栓栓塞症(VTE)的价值。方法收集2008年1月至2017年12月在本院妊娠期合并VTE患者80例的病历资料,与同期产检及住院分娩的无产科合并症孕妇80例为对照组进行对比。比较其血常规、凝血功能与纤溶状态。并结合其临床资料分析其并发VTE的危险度。结果所有观察组病例均经超声显像证实诊断为下肢深静脉血栓形成(DVT)(右下肢23例,左下肢57例)。DVT组孕次及体重指数增加幅度大于对照组(P<0.05);DVT组血红蛋白、红细胞计数、红细胞压积、血小板计数均低于对照组(P<0.05),两组均有高凝与纤溶亢进状态,DVT组更显著(P<0.05)。结论高凝与纤溶亢进状态、稽留流产病史、多孕次、体重指数增长、妊娠期高血压疾病均是VTE形成的危险因素,应多加监测,早期诊断,即时予以防治。
Objective To observe the value of the detection of coagulation function in pregnancy in the prevention of concurrent venous thromboembolism(VTE).Methods The medical records of 80 patients with VTE during pregnancy in our hospital from January 2008 to December 2017 were collected and compared with 80 pregnant women with non-obstetric complications during the same period of birth and hospital delivery.The blood routine,coagulation function and fibrinolytic state were compared.and combined with its clinical data to analyze the risk of its concurrent VTE.Results All the cases in the observation group were confirmed by ultrasound imaging as deep venous thrombosis(DVT)of lower extremities(23 cases of right lower extremity and 57 cases of left lower extremity).The increase of pregnancy and body mass index in DVT group was greater than that in control group(p<0.05).The hemoglobin,erythrocyte count,Hematocrit and platelet count of DVT group were lower than those in control group(p<0.05),both groups had hypercoagulability and fibrinolytic state,and the DVT group was more significant(p<0.05).Conclusion The state of Hypercoagulability and Fibrinolysis,the history of mifepristone abortion,multiple pregnancies,body mass index growth and hypertension in pregnancy are all risk factors for the formation of VTE,which should be monitored,diagnosed early,and immediately prevention and treatment.
