高压氧结合标准大骨瓣减压手术治疗重型颅脑外损伤的效果及对患者预后的影响 被引量:14

摘要 目的:研究高压氧结合标准大骨瓣减压手术治疗重型颅脑外损伤的效果及对患者预后的影响。方法:选取本院2016年1月-2018年5月收治的78例重型颅脑外损伤患者为研究对象,按照随机数字表法分为两组,各39例,对照组予以标准大骨瓣减压手术治疗,研究组予以高压氧结合标准大骨瓣减压手术治疗。比较两组格拉斯哥昏迷量表(GCS)评分、格拉斯哥预后量表(GOS)评分、肌酸激酶(CK)水平,血清C反应蛋白(CRP)水平、动脉收缩期峰流速(Vs)、搏动指数(PI)、平均血流速度(Vm)。结果:干预前两组GCS评分比较,差异无统计学意义(P>0.05),研究组干预后GCS评分及GOS评分均明显高于对照组,差异均有统计学意义(P<0.05)。研究组预后良好率(76.92%)明显高于对照组(43.59%)。差异有统计学意义(P<0.05)。两组干预前Vm、Vs、PI、CK及CRP水平比较,差异均无统计学意义(P>0.05),干预后研究组Vm、Vs均高于对照组,PI、CK及CRP水平均低于对照组,差异均有统计学意义(P<0.05)。结论:高压氧结合标准大骨瓣减压手术治疗重型颅脑外损伤能够促进患者意识恢复,改善脑代谢、血流指标、显著降低其血清CRP水平及CK水平,提高预后质量。 Objective:To study the effect of hyperbaric oxygen combined with standard large bone flap decompression in the treatment of severe extracranial injury and its influence on the prognosis of patients.Method:68 patients with severe extracranial injury admitted to our hospital from January 2016 to May 2018 were selected as the research subjects.They were divided into two groups according to the random number table method,39 cases in each groups.The control group was treated with standard large bone flap decompression,the study group was treated with hyperbaric oxygen combined with standard large bone flap decompression.The Glasgow Coma Scale(GCS)scores,Glasgow Prognosis Scale(GOS)scores,creatine kinase(CK),serum C-reactive protein(CRP),peak systolic velocity(Vs),pulsatility index(PI)and mean blood flow velocity(Vm)were compared between two groups.Result:There was no significant difference in GCS score between two groups before intervention(P>0.05).The GCS score and GOS score of the study group after intervention were significantly higher than those of the control group,the differences were statistically significant(P<0.05).The favorable prognosis rate of the study group(76.92%)was significantly higher than that of the control group(43.59%),the difference was statistically significant(P<0.05).There were no significant differences in Vm,Vs,PI,CK and CRP levels between two groups before intervention(P>0.05).After intervention,the levels of Vm and Vs in the study group were higher than those in the control group,and the levels of PI,CK and CRP were lower than those in the control group(P<0.05).Conclusion:Hyperbaric oxygen combined with standard large bone flap decompression in the treatment of severe extracranial injury can promote the recovery of consciousness,improve brain metabolism,blood flow indicators,significantly reduce the level of CRP and CK in serum,and improve the quality of prognosis.
