从伏风化燥伤阴论治儿童咳嗽变异性哮喘的体会 被引量:18

The treatment of children’s cough variant asthma from the pathogenesis of hidden wind transforming into dryness and consuming yin

摘要 咳嗽变异性哮喘(CVA)是由致敏因素引起的气道反应性增高、咳嗽受体敏感性增高及气道慢性炎症等,临床表现以阵发性刺激性干咳、清晨或夜间发作较多、病情反复、运动后咳嗽明显为特点。从风邪伏肺是基本病机、感受外风为重要诱因、伏风化燥伤阴是主要病机等方面对CVA的中医病机进行探讨,治疗以祛风润燥、养阴敛肺为法。现代研究认为CVA与气道炎症及支气管痉挛密切相关,而中药药理研究提示祛风药具有缓解支气管炎症及痉挛的作用。从伏风化燥伤阴论治儿童CVA取得了较好的临床疗效。 Cough variant asthma(CVA) is caused by sensitization factors, increased airway responsiveness, increased sensitivity of cough receptors, and chronic inflammation of the airways. Clinical expression is paroxysmal irritating, dry cough, early in the morning or at night attack more, repeatedly conditions, and after moving the cough is obvious. From the wind evil stagnation lung is the basic pathogenesis,attacked by the exterior wind as an important incentive, hidden wind transforming into dryness and consuming yin is the main pathogenesis and other aspects of CVA to discuss the TCM of CVA, the treatment is focused on removing wind and moistening dryness, nourishing yin and restraining lung qi. Modern research suggests that CVA is closely related to airway inflammation and bronchospasm, while pharmacological studies of traditional Chinese medicine suggest that the herb of expelling wind has the effect of relieving bronchial inflammation and spasm. It has achieved good clinical results from the treatment of children’s CVA by treatment based upon hidden wind transforming into dryness and consuming yin.
