摘要
目的探讨制霉菌素联合碘甘油治疗白血病患者化疗后真菌性口腔溃疡的疗效。方法选取医院化疗的白血病患者96例,均在常规化疗后发生真菌性口腔溃疡,随机分为对照组与观察组,各48例。对照组每日早、中、晚餐30 min后和睡前给予生理盐水漱口,10~15 ml/次,共4次;观察组将制霉菌素研成粉末后,与20 ml碘甘油充分混合,按照对照组治疗时间,取适量涂抹溃疡创面。连续治疗7 d后,观察比较两组治疗效果及口腔真菌检出率。结果观察组口腔溃疡治疗总有效率为95.83%,显著高于对照组的10.42%(P <0.05);治疗后,对照组口腔真菌感染情况改善不显著,而观察组改善非常显著,且有13例真菌检测阴性,与对照组有显著差异(P <0.05)。结论白血病化疗患者发生真菌性口腔溃疡后,应用制霉菌素联合碘甘油局部涂抹治疗,可显著促进溃疡愈合,并且具有良好的抗真菌作用,可有效改善患者的生活质量,从而减轻患者痛苦,促进患者康复。
Objective To explore the effect of nystatin combined with iodine glycerol on fungal oral ulcer after chemotherapy in patients with leukemia. Methods A total of 96 patients with leukemia receiving chemotherapy in our department were selected and divided into a control group and an experiment group randomly and equally(n=48, respectively). The patients all experienced fungal oral ulcer after routine chemotherapy. The patients in the control group were administered with normal saline to rinse the mouth 30 minutes after breakfast, lunch and dinner and before sleep every day(10-15 ml/time;four times in total). In the experiment group, the nystatin was ground into powder and then well mixed with 20 ml iodine glycerin;the mixture was applied to the ulcer surface as per the administration timing in the control group. All patients in both groups were observed for the healing condition and detection rate of oral fungus after seven days of continuous treatment. Results The total effective rate in the experiment group was significantly higher than that in the control group(95.83% vs. 10.42%;P < 0.05). After treatment, the fungal oral infection in the experiment group was significantly improved with 13 negative results of fungus, quite opposite to the control group. There was a significant difference between the two groups(P < 0.05). Conclusion Topical application of the nystatin combined with iodine glycerol to the ulcer surface can be associated with faster healing of ulcer, reduced fungal infection, improved life quality, alleviated pain, and early recovery of patients who experienced fungal oral ulcer after chemotherapy.
